The primary care services we provide vary from specialist services, providing care to vulnerable and homeless populations, to more mainstream general practice and we work across East London and Bedfordshire. The Long Term Plan sets out a vision for primary care to lead on population health improvement and we work in partnership with mental health, community services, acute services, social care, the voluntary sector and ambulance services to help our local populations stay well.

Main duties of the job

  • The post holder will be an experienced nurse or allied health professional, who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice.   
  • Non-Medical prescriber
  • The post holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
  • The post holder will build effective working relationships with other health care professionals.
  • The post holder will work within the multi-disciplinary team to deliver excellent primary health care services within the practice
  • Clinical Expertise: Demonstrate expert clinical knowledge and skills in the assessment, diagnosis, and management of patients. This includes conducting comprehensive patient assessments, formulating treatment plans, and prescribing medications as authorised.
  • Collaborative Approach: Work collaboratively with the healthcare team to provide holistic care and ensure effective coordination of patient services. This involves regular communication, consultation, and collaboration with physicians, nurses, and other healthcare professionals

Detailed job description and main responsibilities

  • Clinical Leadership: Provide clinical leadership and guidance to junior staff members, including mentoring, supervision, and support. Foster a culture of continuous learning and professional development within the healthcare team.
  • Research and Evidence-based Practice: Stay up-to-date with the latest research and evidence in the field of clinical practice. Apply this knowledge to enhance patient care, contribute to research activities, and participate in quality improvement initiatives. Quality Improvement: Identify opportunities for quality improvement within the healthcare setting. Contribute to the development and implementation of protocols, guidelines, and best practices to enhance patient outcomes and safet
  • Patient Education: Educate and empower patients and their families by providing clear and concise information about their condition, treatment options, and self-management strategies. Foster a patient-centred approach that promotes shared decision-making and patient satisfaction.
  • Professional Collaboration: Engage in interprofessional collaboration and contribute to multidisciplinary team meetings, case conferences, and clinical audits. Act as a resource and liaison for other healthcare professionals to optimise patient care.
  • Documentation and Record-keeping: Maintain accurate and up-to-date patient records, including electronic health records, progress notes, and treatment plans. Ensure compliance with legal and regulatory requirements regarding patient confidentiality and data protection

Education, Training, Qualifications

Essential criteria
  • 1st Level NMC Registration
  • Advanced clinical Practitioner Master’s Degree
  • Evidence of continuous learning and professional development
  • Significant recent experience in General Practice and primary care
  • Experienced and competencies in the management of Long term Conditions
  • Responsible for and Evidence of Completed Clinical Audit
  • Experience of line managing other nurses
  • Excellent commination skills, verbal and written
  • Understands legislation and government initiatives particularly in relation to Primary Healthcare and Nursing
  • Ability to work in a multi-disciplinary team
  • Experience of leading on the delivery of QOF in General Practice
Desirable criteria
  • Level 3 Reproductive Health (ENB 901 / 8103) or equivalent
