Job overview
The Advanced Clinical Practitioner (ACP) is a highly skilled healthcare professional responsible for delivering expert, autonomous patient care in a primary care setting. The ACP will assess, diagnose, treat, and manage patients with acute and chronic conditions, working collaboratively with GPs and the wider multidisciplinary team.
Key Responsibilities:
- Conduct clinical assessments and make independent treatment decisions.
- Diagnose and manage long-term and urgent conditions.
- Prescribe medications and initiate treatment plans.
- Support service development and clinical governance.
- Provide leadership, education, and mentorship to junior staff.
Main duties of the job

Clinical Assessment & Diagnosis
- Conduct comprehensive patient assessments, including history-taking and physical examinations.
- Diagnose and manage acute and chronic conditions, initiating appropriate treatment plans.

Treatment & Prescribing
- Prescribe medications (if qualified with V300) and initiate treatments within the scope of practice.
- Perform clinical procedures and interventions as required.

Patient Management & Care Planning
- Develop, implement, and evaluate personalized care plans.
- Provide follow-up care and refer patients to appropriate specialists when necessary.

Multidisciplinary Team Collaboration
- Work closely with GPs, nurses, paramedics, and other healthcare professionals to ensure integrated patient care.
- Support and contribute to the development of primary care services.

Clinical Governance & Service Improvement
- Participate in audits, quality improvement projects, and risk management activities.
- Ensure compliance with clinical guidelines and best practices.

Leadership, Teaching & Mentorship
- Supervise and mentor junior staff, trainee ACPs, and students.
- Lead training sessions and contribute to staff development.

- Provide rapid assessment and management of urgent cases.
