Job overview

  • The post holder will provide advice and support to Staff within Tower Hamlets on the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. 

  • Be responsible for undertaking and quality assuring NHS Continuing Healthcare Assessments with members of the Multidisciplinary Team (MDT), ensuring that cases are presented to panel within the specified time frame. 

  • Undertake reviews for NHS Continuing Healthcare and NHS Funded Nursing Care Patients and ensuring that they are completed within a timely manner. 

  • To case manage Patients who meet the criteria for NHS Continuing Healthcare.

Main duties of the job

  • To act as Lead Coordinator for NHS Continuing Healthcare Assessments and undertake comprehensive, robust and skilled health assessments for Patients with the Multidisciplinary Team. 

  • To agree a shared care plan with members of the Multidisciplinary Team for Patients who are assessed as meeting the criteria for shared care with the Local Authority, ensuring robust care plans are in place to support the decision. 

  • To case manage Patients who meet the criteria for NHS Continuing Healthcare. To monitor the standard of care provided and coordinate, liaise with Health and Social Care Professionals. 

  • To provide advice and support to the MDT regarding NHS Continuing Healthcare Assessments, provide quality assurance on behalf of the Clinical Commissioning Group (CCG) and ensure that the Patient’s case is discussed at Continuing Healthcare Panel within 28 days of receipt of referral. 

 

  • Attend NHS Continuing Healthcare Panel and present cases for Patients assessed under the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. 

 

  • To arrange Care Packages/Placements for Patients deemed eligible for NHS Continuing Healthcare in conjunction with Commissioning. 

  • To arrange Case Conferences and attend Safeguarding Meetings as necessary. 

  • To monitor and prioritise own workload in collaboration with Lead Nurse and Senior Staff within the CHC Team.

    • To allocate case load to Junior Staff members, ensuring equitable distribution of case load and timely assessment of Patients, re-priortising own workload if required. 

    • To provide effective mentorship for Student Nurses & Junior Staff members enabling them to maximise their own potential. 

     

    • To appraise and provide clinical supervision for Junior Staff. 

    • To undertake clinical supervision as a means of reflective practice and maintain a personal development portfolio in accordance with trust guidelines. 

    • To participate in any Disciplinary or Grievance Procedure. 

    • To support the Lead Nurse by participating in formulating, updating and reviewing all policies and procedures as appropriate and relevant to the area of practice. 

    • To ensure that trust policies and procedures are maintained at all times, confidentially being paramount. 

    • To assist in other areas as required by Lead Nurse. This may include occasionally working outside of your current role and in different work locations. 

    • To attend meetings as appropriate and represent the Continuing Healthcare Team as requested by Lead Nurse

Education/ Qualification/ Training

Essential criteria
  • 1st Level Registered Nurse (RGN/RMN)
  • Evidence of Continuous Professional Development
  • ENB Certificate 998 Teaching and Assessing/ Preparation for Mentorship Qualification or equivalent qualification
  • Educated to Degree Level or equivalent
Desirable criteria
  • Specialist Practitioner District Nurse
  • Leadership/management course

Skills, Knowledge and Experience

Essential criteria
  • Experience in conducting comprehensive health assessments
  • Experience in providing teaching and support to other clinical staff.
  • Experience working as an autonomous practitioner.
  • Demonstrate experience of Multi –Agency working across Health & Social care
  • Knowledge & experience of Patient’s information systems e.g. EMIS
  • Experience of using electronic systems such as Microsoft Word & Excel
  • Knowledge of National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care.
  • Able to produce concise and accurate verbal and written reports in accordance with the NMC Code of Conduct and NHS policy and protocols.
  • Knowledge and understanding of the philosophies of the Safeguarding and the protection of vulnerable Adults
