Job Purpose

The Clinical Nurse Specialist (CNS) in Palliative Care will work to empower people with advanced progressive illnesses and those who support them to maximise their individual quality of life and death by enabling the provision of skilled palliative care.


Key Working Relationships


Specialist Palliative Care services in Harrow and NWL, particularly at St Luke’s Hospice, acute based Palliative Care and Macmillan services, General practitioners, adult community service providers including other specialist teams and district nursing, social services.

Main duties of the job

The CNS in Palliative Care works as a member of Harrow Community services within Central London Community Healthcare NHS Trust. The team is comprised of a nurse team leader, 6.3 WTE clinical nurse specialists, (one in a development role) one full time specialist palliative care occupational therapist, 3 part time consultants in Palliative Medicine and 1.2 WTE team administrators.


Acting as a specialist resource to provide advice, support and information to patients, families and staff, the CNS plays a key role in raising clinical standards in the provision of palliative care services in the community. The CNS will act as a role model and exercise leadership to influence and develop the practice of nurses, medical and social care personnel in palliative and end of life care.

 In collaboration with the Palliative Care Consultants and other members of the specialist multi-disciplinary team (MDT) the post holder will provide expert advice on pain, symptom control and the ongoing management of patients with specialist palliative care needs.


The post holder will work within the Harrow Community Palliative Care Service, serving the residents of the borough of Harrow, which has over 240,000 residents. Harrow is one of the most ethnically diverse boroughs in England and Wales (Office of National Statistic) with over 60% of the population being from minority ethnic group

Education/Qualification

Essential criteria
  • First level nursing registration
  • First level degree in related subject or evidence of work towards this
  • Teaching and assessing qualification or experience.
  • Advanced Communication Skills Course
  • Level 2 psychology training.
Desirable criteria
  • Non-Medical Prescriber V300
  • Qualified MSc

Experience

Essential criteria
  • Relevant post registration experience at band 6 or above in this or appropriate speciality
  • Previous experience in a post involving clinical and managerial responsibilities.
  • Previous experience of teaching and developing teaching materials
  • Demonstrates evidence of well-developed clinical practice
  • Ability to deal with complex patient care including symptom control and psychological support
  • Able to manage own caseload
Desirable criteria
  • Conversant with the key principles of clinical governance

Skills and Knowledge

Essential criteria
  • Ability to deal with complex patient care including symptom control and psychological support
  • Ability to communicate unpleasant /difficult information to patients in a variety of settings with sensitivity.
  • Able to perform assessment, planning, implementation and evaluation of nursing care.
  • Knowledge of professional and NHS issues, and policy relating to specialist area.
Desirable criteria
  • Able to influence management and overcome resistance through application of advanced communication skills.
