We are enhancing our Urgent Community Response Team and have opportunities within the team for a senior clinician. The post holder will be a senior member of the team. The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Team, other health care professionals and statutory and voluntary agencies to deliver excellent standards of patient care.

The successful candidate will be an experienced Practitioner with sound clinical knowledge and proven experience in acute and community clinical environments. You will need to be adaptable to operate in a fast-moving environment providing unscheduled holistic clinical care to patients presenting with acute undifferentiated and undiagnosed conditions, complex health needs and /or long-term conditions.

 

Main duties of the job

As senior practitioner you will act as role model supporting the Urgent Community Response team to achieve and maintain high standards of clinical care, based on evidence-based practice.

As Senior Urgent Community Response practitioner you will support the clinical lead to respond to clinical incidents, accidents and complaints in accordance with CLCH policy.

Work in collaboration with and maintain a good working relationship within the other Community Adult Health Care Teams within the borough of Harrow its adjoining boroughs, and with the District nursing team, Out of Hours Services, Accident and Emergency Departments, London Ambulance Service, Acute Trust, Community Matrons, General Practitioners and Practice staff, Nursing Homes, Social Services and Voluntary Sector so as to provide a comprehensive service to patients, clients and carers.

Undertake assessment and appraisal of designated members of the team.

Education/Qualification

Essential criteria
  • Health care professional with current registration ( Nurse ) or similar
  • BSc / Pg Dip Nurse Practitioner Primary Health Care/ Minor injuries / Minor Illnesses
  • Experience in Community and/ or Accident and Emergency
Desirable criteria
  • Evidence of continuing professional education and training at master’s Level.
  • Further desirable qualifications in Primary Care/ Accident & Emergency/Advanced clinical practitioner
  • Qualification in teaching or practice-based learning.
  • Independent prescriber or willingness to undertake

Experience

Essential criteria
  • Extensive experience in Community, Primary Care and/ or Accident & Emergency.
  • Proven experience of utilising expert diagnostic and clinical skills to provide autonomous, unscheduled medical care and management of patients presenting with undifferentiated and undiagnosed conditions.
  • Experience of assessing and providing quality care to people with Long Term Conditions
  • Track record of practice excellence.
  • Experience of effectively working in collaboration, negotiating and liaising across organisational boundaries.
  • Experience in monitoring clinical standards.
  • Proven ability to prioritize and manage own workload with minimal supervision
  • Deputize of clinical lead in their absence
  • Experience of team / service/ staff management.
  • Experience of implementing evidence-based care and developing others to do so.
Desirable criteria
  • Experience of conducting research projects and audit.
  • Experience of change management and implementing change in practice.
  • Evidence of innovation in practice

Skills & Knowledge

Essential criteria
  • Evidence of comprehensive range of clinical skills and expertise relevant to the role
  • Competent in physical examination of: ENT, eye, gastrointestinal, respiratory, cardiovascular, neurological, skin, musculoskeletal systems and wound management.
  • Basic IT skills and experience of using Cerner, EMIS and Email
  • Specialist knowledge in clinical reasoning, history taking, physical examination, diagnostic skills and prescribe or derive clinical management plan
  • Knowledge of pharmacology
  • Advanced knowledge in in depth understanding of anatomy and physiology
  • Knowledge of MSK presentations and management of acute conditions.
  • Able to assess for in home safety, equipment needs and order as per local guidelines.
  • Clinical knowledge across a range of long term and enduring conditions
  • Knowledge of current NHS organizational structures and policy initiatives.
  • Knowledges of recent developments in Primary care
  • Knowledge of monitoring service outcome measures.
  • Knowledge of Adult protection and safeguarding Childrens issues.
  • Knowledge of NMC code of conduct and/ or other professional regulatory bodies.
  • Knowledge of personal development planning.
  • Knowledge of current health care policy and legislation and relevance to the delivery of healthcare services.
  • Detailed knowledge of Primary Health Care and collaborative working with other care providers e.g social care and Voluntary services
  • Knowledge of the audit process and able to implement in practice.
  • Knowledge of clinical governance processes, assessing and managing risk.
  • Experience in clinical supervision sessions
