Job overview

We are looking for a motivated individual, with a strong understanding of safeguarding, and who understands the complexities of undertaking assessments in adults who have experienced violence. A positive, can do 'attitude', a commitment to changing care for this extremely vulnerable patient group, and a desire to communicate with a wide variety of partners and stakeholders is essential.

Main duties of the job

The Band 6 Serious Violence Practitioner will hold a caseload and work with the patients in a range of settings, including face-to-face in the emergency department, on hospital wards, via telephone, in group contexts and in the community. They will promote their personal, health and social development by undertaking a holistic assessment and supporting a personalised care plan.

Detailed job description and main responsibilities

On a day-to-day basis the Serious Violence Practitioner is responsible for the clinical governance of the service and identifies areas for quality improvement and ensures adverse trends are identified, reported and managed. This will involve collecting data and being involved in shaping the projects future.

 

The Serious Violence Practitioner is a key player in delivering system change within the organisation including taking a lead on teaching around inclusion health and violence within the organisation. This expectation for this role would be on site with community outreach as needed.

In detail the service aims to:

 

·         effectively engage the patient in all relevant services / support,

·         maximise the benefit of the attendance / admission for the patient from a health and social care perspective,

·         link the patient into all necessary health and social care and support pending discharge,

·         advocate for the patient to receive assessment, treatment or services when this is required

·         safeguard patients effectively

·         ensure a safe and effective discharge to accommodation (where this is possible)

·         stop the revolving door

·         reduce re-offending

·         reduce and prevent violence

·         support those vulnerable of violence

Qualifications

Essential criteria
  • Relevant qualification or comparable experience

Experience

Essential criteria
  • Significant experience of working with vulnerable adults in a supporting, advisory or advocacy role
  • Extensive experience in a relevant discipline
  • Experience of successfully managing a fast-moving case load that involves triage and prioritisation
Desirable criteria
  • Experience of joint working with healthcare services, or working in healthcare settings
  • Experience of setting up, and maintaining relationships with a wide variety of relevant community partners

Skills

  • Excellent oral and written communication skills
  • Confident to problem solve and make independent decisions
  • Team worker
Desirable criteria
  • Demonstrated ability to work successfully with patients with personality disorder and complex trauma

Knowledge

Essential criteria
  • Good knowledge of the Serious Violence Duty
  • Good knowledge in inclusion health
  • Good knowledge of how to work successfully with patients with communication barriers e.g. language, literacy, cognition, behaviour
Desirable criteria
  • Leadership training
  • Knowledge regarding the support options available for people experiencing violence and/or homelessness in the local community
