Highly Specialist Speech and Language Therapist (Stroke)

Job overview
This is an exciting opportunity for a Band 7 post in the Early Supported Discharge Team with Stroke in Hillingdon. Its a part time post based in Hillingdon providing intensive speech and language therapy to people who have had a stroke. The SLT service is provided from Monday to Friday for up to 6 weeks following early discharge from hospital. All care is provided to patients in their own homes.
You will be experienced in management and therapy for aphasia, dysarthria and dysphagia following stroke. You will work closely as part of a multi-disciplinary team of consultant , physio, OT and rehabilitation support workers as well as reablement carers. The ESDT for Stroke is a joint enterprise between CNWL NHS Foundation Trust, Hillingdon Hospital and social services.
You will be working alongside other speech & language therapists, who provide longer-term care for adults with acquired disorders of communication and swallowing on an out-patient and domiciliary basis.
Competence in dysphagia is essential and the ability to undertake video fluoroscopy assessment would be an advantage. The role is suitable for any experienced Band 6 looking for an opportunity to progress to a Band 7.
The service provides SLT to adults with acquired disorders of swallowing and or communication across the London Borough of Hillingdon who are in the community. There are regular team meetings, good peer support with robust clinical supervision and excellent opportunities for training.
Main duties of the job
- To work as a highly specialist member of a multi-disciplinary team delivering a service to adults who have had a stroke in their home settings.
- Acts as lead clinician for the SLT service within the ESD service , by identifying, implementing, and monitoring service development in this field, working closely with the occupational therapist, physiotherapists and Rehab support workers.
- Provide highly specialist assessment, diagnosis and appropriate speech & language therapy for adults with eating, drinking, swallowing and communication difficulties.

Detailed job description and main responsibilities
Principle responsibilities
- To monitor and develop provision of the SLT service within ESD
- To monitor and lead on the training and supervision of other SLTs who are developing their skills in working with stroke.
- To undertake other duties, as requested by the line manager to ensure the efficient, effective running of the SLT Service.
Clinical Duties
- To assess, diagnose, treat and manage independently a highly complex and diverse dysphagia caseload, using instrumental measures (videofluoroscopy, FEES, sEMG) where appropriate, undertaking training in these where necessary. · To assess, diagnose, treat and manage independently a complex and diverse caseload of people with speech, language and communication difficulties
- To work as an influential and effective member of the Speech and Language Therapy team.
- To participate in inter-disciplinary assessment of clients’ and carer’s needs and priorities.
- To provide information and highly specialist advice to clients, carers and professionals.
- To ensure clients are involved in the planning and prioritization of their care plans across the service.
- To participate in multi-disciplinary planning of intervention / management to meet client’s identified goals.
- To provide highly specialist intervention and evaluate outcomes.
- To write reports reflecting highly specialist knowledge.
- To negotiate with other professionals, families and carers around case management in complex cases.
- To adapt practice to meet individual clients’ circumstances, including due regard for cultural and linguistic differences.
- To use and promote the use of evidence based practice across the S< Service.
- To deal skilfully with complex issues and generate appropriate strategies for caseload management of self and others.
- To provide second opinions on request for complex clinical cases across the adult SLT service.
- To provide appropriate support and supervision to other therapists across the adult SLT service.

Qualifications
Essential criteria
- Recognised Speech and Language Therapy Degree or equivalent
- Registered with Health and Care Professions Council
- Evidence of successful completion of specialist short courses up to Master’s degree equivalent
- Membership of relevant Clinical Excellence Networks
Desirable criteria
- Registered Member of Royal College of Speech and Language Therapist
Experience
Essential criteria
- Considerable experience working with adults with acquired disorders and a significant proportion of this should be with stroke.
- Relevant highly specialist experience post qualification in assessment and management of acquired communication/swallowing disorders of neurological aetiology
- In-depth experience within specialist area of acquired neurological disorders of communication and swallowing due to stroke
- Involvement in multi-disciplinary working
Desirable criteria
- A significant proportion of post graduate experience should include community setting.
- Independent in videofluoroscopy
- Experience of supervising and appraising more junior therapists
- Experience working with interpreters
- Experience of developing and providing specialist training to carers, and other members of the MDT
Knowledge and skills
Essential criteria
- Excellent presentation skills, both written and verbal
- Excellent caseload management skills, including the ability to prioritise caseload and work independently
- Demonstrates ability to be a good team member and assume leadership responsibilities
- Well established knowledge of a broad range of assessment tools relevant to adult voice, communication and swallowing disorders
- Knowledge of high and low technology communication aids and their use with adult neurological patients
- In depth knowledge of a range of appropriate therapeutic interventions relevant to communication and dysphagia in specialist patient group and an ability to compare and contrast relative benefits
- In depth knowledge of the principles of clinical governance /audit / record keeping
- Knowledge of up-to-date advances in clinical management relevant to specialist patient group and application to own clinical practice
Desirable criteria
- In-depth knowledge of national policies and procedures relevant to specialist patient group
