Job overview

This is an exciting opportunity for an experienced, patient-focused clinical pharmacist with relevant specialist knowledge and good management and leadership skills. The post-holder will manage the Pharmacy staff 
resource and service provision and lead in all aspects of medicines optimisation to the elderly care and stroke/neuro rehabilitation specialist areas across the Trust.

Attention to detail as well as the ability to see the wider picture is called for in this challenging and demanding role. The post-holder will further develop our well-established role in the transfer of care around medicines and 
the Discharge Medicines Service. Ensuring the safe discharge of our patients to primary/community care will also afford opportunity to develop close working relationships with our Integrated Discharge Team and CCG, 
PCN and community pharmacy colleagues.

The Pharmacy Department provides a reliable, quality, patient centred service with empathy, enthusiasm and commitment. It prides itself in being forward thinking and keeping pace with changes in medicines optimisation 
and technology. We are well respected throughout the hospital and have a close working relationship with the clinical teams. We also have a high level of commitment to staff development and actively encourage all staff to 
share their expertise to enhance the performance of the department.

 

Main duties of the job

The post holder is an experienced clinical pharmacist with specialist knowledge, highly developed management, and leadership skills, who will manage the Pharmacy service provision and lead the medicines optimisation agenda to medical elderly care, frailty and 
rehabilitation wards and departments. This includes: 

1. Ensure compliance of self and team members, with medicines legislation, policies and procedures

2. Ensure a safe, legal and timely supply of medication to patients

3. Work independently and also liaising closely with the other Senior Pharmacists, Principal Pharmacists, Chief Pharmacist and Chief Technicians

4. Be responsible for the day-to-day planning and provision of the clinical pharmacy and medicines management services within his/her directorate and to ensure timely delivery of services in line with key performance indicators

5. Positively influence cost effective prescribing 

6. Assist in the development of clinical guidelines, protocols and Patient Group Directions (PGDs)
