This opportunity will see you work closely with multi-organisation partners to improve the patient experience, whilst facilitating appropriate discharge destinations.
Main duties of the job
The DTA Beds Therapy Team provides ongoing therapy and facilitates discharges. This includes managing caseloads across multiple care homes in South and West Hertfordshire. We also complete home visits to organise equipment for discharge home all in a timely manner with the assistance of social services.
We are looking to recruit an experienced Physiotherapist to co-lead the Discharge to Assess (DTA) bed therapy service within West Hertfordshire. If you have experience of leading within health services and have a desire to further develop a service in a highly performing, stimulating and fast-paced environment then this may be your opportunity.
You will be fully supported by the team and service manager as you grow your knowledge of clinical services within West Hertfordshire and beyond to ensure patients receive the most appropriate care in a timely manner.

Qualifications
Essential criteria
- Current AHP with UK HCPC registration
- Diploma/degree in Physiotherapy
- Demonstration of innovation in clinical practice
- Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation or older people
Desirable criteria
- Future education in Clinical or Management Studies or equivalent
- Post graduate degree in relevant area
- Member of a Special Interest Group relevant to this post
- Member of CSP
Experience
Essential criteria
- Experience working at a senior level in a wide range of clinical areas including, neurology, general rehabilitation, falls, elderly medicine.
- Able to communicate with patients, relatives and MDT and on occasions deal with conflicting views
- Experience of working in the NHS
- Evidence of service improvement within previous role
Desirable criteria
- Experience of wide range of specialist services eg. falls services, wheelchair services, and learning disabilities.
- Experience working as a Band 6 Physiotherapist
- Experience of community rehabilitation
- Knowledge of common causes of delays which lead to a delayed transfer of care (DTOC) for patients
Skills
Essential criteria
- Skills in the assessment and treatment of adults with complex therapy needs
- Knowledge of the local health and social care system
- Knowledge of NHS reforms and key local and national policy drivers
- Knowledge of acute hospital settings and where common delays to the transfer of care may occur
- Communicate effectively at all levels including well developed verbal and written communication skills
- Computer literacy
Desirable criteria
- Knowledge of community-based therapy pathways and rehab units
- Knowledge and understanding of National Service Framework for Older People, NICE Guidance and Clinical Guidelines
- Knowledge and understanding of NICE and CSP/RCOT Fallers Guidelines
Key Attributes
Essential criteria
- Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
