Job overview
JOB PURPOSE
• The Health Care Assistant will provide a mixture of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional. The HCA will work in patients own homes.
• The post holder will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The HCA will promote optimum independence and individualised client centred care to a delegated caseload.
• The post holder will also support the unplanned care team to deliver against key performance indicators and maintain quality of the service. The service is provided seven days per week 8pm-8am covering the four localities across Herts Valleys.
Main duties of the job
To contribute to the on-going assessment of a patient by completing the appropriate paperwork for allocated patients, recording, and reporting all clinical and behavioural information to the nurse or therapist for action and planning purposes.
To undertake, and report back on delegated clinical activities that have previously been signed competent. Eg: vital signs, routine urinalysis, glucose monitoring.
To enable patients/carers to make most effective use of resources within the service by ensuring they receive up-to-date information sheets, responding to any questions constructively
To pass onto/seek assistance from other colleagues, promptly, any questions that you are unable to resolve

Education/Qualification
Essential criteria
- Care qualification to NVQ 3 or equivalent experience.
- GCSE English and Maths grade 9-4 or equivalent.
Desirable criteria
- Higher Professional Diploma in Health and Wellbeing (NVQ/QCF – Level 3) or a recognised equivalent course.
Experience
Essential criteria
- Experience of working in a health or social care setting
- Experience of working as part of a team
- Experience with the use of Microsoft Office and a variety of databases.
Desirable criteria
- Experience of working as part of a team
- Experience with the NHS patient information database (SystmOne)
Skills and Knowledge
Essential criteria
- Knowledge of NHS
- Demonstrate an acceptable level of numeracy and literacy.
- An awareness of risk management.
- Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
- Excellent communication skills and ability to build effective relationships
- Ability to analyse, interpret and record data in a clear, and succinct manner
- Excellent oral and written communication and presentation skills

- • Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
