District Nurse Team Leader - Band 7
An exciting opportunity has arisen for a Band 7 Team Lead to join our Community Adult Planned Care Home Team. The team consists of Community Nurses, District Nurses, Nursing Associates and Health Care Assistants. The team covers 8am-6pm, 7 days a week supporting care homes across all 4 localites in Watford, Dacorum, Hertsmere & St Albans and Harpenden. Our aim is to be the best we can, offering safe, patient-centred and evidence-based care to those accessing our service.
We are looking to recruit an enthusiastic and motivated Team Leader to join the Planned Care Team. The post holder will provide leadership working alongside the other Team Leads to support the community nursing team to provide excellent, evidenced based care to the patients on the caseload.
The post holder will be part of the leadership team but must maintain clinical competencies, hence a significant proportion of your hours will be clinical. This is to ensure that those patients with complex needs are managed effectively and the team members are supported to develop their skills and competencies.
Main duties of the job
The successful candidate will have the opportunity to develop their clinical leadership and operational management skills, taking responsibility for a range of tasks to support the smooth running of the service and the delivery of high quality clinical care.
You will be supported by the Locality Manager and will have the opportunity to develop strong peer relationships with colleagues from the wider leadership team for the Planned Care Clinical Business Unit across all localities in West Herts.

Education/Qualification
Essential criteria
- RGN / RN with current registration
- Evidence of continuing professional education and training at degree level
- Mentor or practice based learning training
- Community or Independent Nurse Prescribing qualification or prepared to work towards
Desirable criteria
- Post –registration qualification in district nursing or equivalent
- Qualification in team / service management or prepared to work towards.
Experience
Essential criteria
- Evidence of recent post-registration nursing experience in community nursing
- Experience of negotiating and liaising within a multi-disciplinary team
- Experience of working with people with complex needs
- Experience of co-ordinating and managing a team
- Experience of teaching others and mentoring students
- Experience of evidence-based care
- Experience of undertaking comprehensive health needs assessment and developing complex care plans
- Experience of co-ordinating and delivering care for people with complex, long term conditions.
Skills and Knowledge
Essential criteria
- Knowledge of NMC Code of Professional Conduct.
- Knowledge of current health care policy and relevance to the delivery of healthcare services
- Detailed knowledge of Primary Health Care and collaborative working with other care providers, e.g. social care and voluntary services
- Knowledge of clinical governance
- Knowledge of clinical supervision
- Possesses clinical skills and expertise relevant to the role
- Ability to communicate effectively both verbally and in writing
- Basic computer skills
Other
Essential criteria
- Able to carry out the duties of the post With or without reasonable adjustments
- Able to travel around the district (car/bike/public transport/foot)
- Hold a clean UK drivers licence with access to a car (Barnet only)
Desirable criteria
- Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
