We are seeking a dynamic and enthusiastic Specialist Grade Doctor to work in our Older Adults Frailty Team. This is a new multidisciplinary mental health team, which will work alongside the Harrow OA community Mental Health Team, Memory Service, and the Older Adult Home Treatment Team. You will receive supervision from the Older adult consultant psychiatrist.

In CNWL Harrow we have an ethos of whole systems working closely together. The service has strong links to Harrow Mental Health Services, including CNWL Community Mental Health Services and Adult Home Treatment Team in Harrow, Older Adult Liaison Services at Northwick park hospital and the Ellington ward which is the acute older adult psychiatric ward.
In line with the NHS 10-year long term plan, CNWL Harrow has embedded the Trauma Informed Approach. All staff receive regular training and supervision in both formulation and treatment, using this approach. The care is delivered by an integrated multi-disciplinary team with a shared sense of ownership for patients.

Detailed job description and main responsibilities
• To be responsible for carrying out detailed psychiatric assessments and risk assessments and to contribute to planning and implementing service users’ care plans.
• Attend the frailty MDT
• Provide input regarding treatment of psychiatric illness in community frail patients. This includes dose titration and risk management advice.
• Diagnose dementia in community frail patients. Initiation and supervising of dementia medication.
• Support frailty team in the differentiation of delirium from dementia.
• Management of BPSD in frail patients.
• Assist in management of agitated older patient on frailty team.
• Advise GP regarding management of depression in primary care.
• Ensuring pathway of community engagement information to local, cultural and faith groups to be delivered in the community.
• working with the current GP led frailty service to reduce admission and attendance at the A&E
• To carry out the above in a variety of settings, including home visits
• To work flexibly to ensure that clients and their carers can access the service effectively.
• To provide the day-to-day medical cover to the Frailty Team, with supervision from the Older Adult Consultant.
• To review clients within the community as well as those who require a hospital admission for medical assessments and evaluation including physical and mental state examinations.
• To participate in the Middle Grade on-call Rota for Harrow. Currently the frequency of on call is 1:10, but this may vary in the future according to service needs.
• To be an active member of the multi-disciplinary team.
• To involve the user and carers in the treatment, management and care planning of a user’s mental health and physical care.
• To liaise effectively with appropriate agencies, statutory and non-statutory, other mental healthcare providers, GPs, families, and carers, as appropriate, regarding clients.
• To ensure that clinical records are kept up to date, accurate, signed, and legible.
• To ensure that all aspects relating to discharging clients are appropriately dealt with, and all clients are diagnosed with ICD10 code.
• To take an active part in devising and reviewing clinical protocols.
• To participate in Continuing Professional Development.
• To participate in annual appraisal in line with Trust policy, from the Consultant or Lead clinician.
Person specification
Education and Qualifications
Essential criteria
- MB BS or equivalent medical qualification
- Full GMC Registration with a license to practice
- To be section 12(2) MHA 1983 approved (or achieve within 6 months of apt), if not have Approved Clinician (AC) status/approval and be on the AC Register
- A minimum of six years should have been in a relevant specialty in the Specialty Doctor and/or closed SAS grades.
- Equivalent years’ experience in a relevant specialty from other medical grades, including from overseas.
Desirable criteria
- MRCPsych (or equivalent)
- An additional post graduate qualification of relevance to (own specialty)
Experience
Essential criteria
- Proficient in risk assessment and management
- Experience of working in multidisciplinary teams
- Experience of use of the Mental Health Act and Mental Capacity Act in an inpatient psychiatric setting
- Demonstrable leadership skills
- Adequate training in child and adult safeguarding
- Demonstrates ability to challenge others, escalating concerns when necessary.
Desirable criteria
- Experience of service development/business planning
- High level knowledge of planned developments at a national level to improve efficiency and deliver quality services

- Awareness of legal responsibilities relevant to the role, such as around mental capacity and deprivation of liberty; data protection; equality and diversity.
