Job overview
Robertson Ward is a 20 bedded ward. Consultant lead specialist neurological rehabilitation in-patient ward. We provide multidisciplinary rehabilitation for patients with complex disabilities which are neurological in origin, including conditions such as Stroke. Multiple Sclerosis, Traumatic Brain Injury. Parkinson's Disease, Motor Neurone Disease and Spinal cord Injures.
We are an adult service providing rehabilitation to individuals aged range 16 to 64. However, older patients than 64 are now accepted if considered appropriate following the team's assessment.
We are seeking a motivated, caring, and dynamic rehabilitation nurse with experience in stroke to apply for this exciting opportunity to work in our team.
The rehabilitation Nurse will work closely with the Multidisciplinary Team consisting of Consultants and Doctors, matrons, ward Manager, Staff Nurses Healthcare assistant, Therapists, Dieticians, Pharmacists, Psychologists and Domestics staff.
The rehabilitation Nurse will assist the Ward manager to provide nursing care to, and support recovery of patients to meet their rehabilitation goals, focusing on the needs of the patients rather than the illness or condition. This will be an opportunity to work directly with patients which are neurological origin.
Furthermore, the rehabilitation nurse will work closely with other Allied Health Professionals and Social Care Services, to maximise patient's recovery.
Main duties of the job
To provide skilled, effective and evidence based nursing care to patients in the Neurological Rehabilitation ward.
To identify, prioritise and implement of care plans that meet the needs of the patients.
To promote the participation of patients and relatives in assessments and care planning.
To ensure that all care plans in their unit are fully completed, up-to-date and evaluated regularly.
To ensure each patient has a comprehensive risk assessment and management plan documented in the care plan.
To support junior members of staff to complete, implement and evaluate care plans.
To participate in, and help others to participate in, regular multi-disciplinary clinical reviews.
To identify and record all new problems and concerns and report back to the Ward Manager.
To ensure the safe handling of body fluids and contaminated sharps.
To provide specialist nursing skills and knowledge such as continence assessment and management, falls assessment and prevention, pressure sore prevention and management and medicines management.
To undertake MUST nutritional assessments and advise and support patients with their nutritional needs e.g. PEG feeds.

Education/Qualification
Essential criteria
- Registered Nurse with current UK NMC registration
- Post –registration qualification in older people’s nursing or equivalent.
- Evidence of continuing professional education and training at diploma/ degree level
- Qualification in team / service management or prepared to work towards.
- Mentor or practice based learning training
Desirable criteria
- Evidence of recent post-registration nursing experience in older peoples’ nursing.
Skills and Knowledge
Essential criteria
- Knowledge of NMC Code of Professional Conduct.
Desirable criteria
- Able to work autonomously, planning and prioritising own workload and that of others
Applicant requirements
The postholder will have regular contact with vulnerable people and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
