The post holder will need a positive and can-do attitude with the ability to effectively prioritise and manage a conflicting workload. The post holder must be able to confidently work unsupervised and as a team to meet targets and deadlines. The post holder will have a good understanding of the National Framework, mental capacity act and case law around continuing healthcare.
Main duties of the job
You will ensure patients go through the CHC process efficiently. You will communicate and develop strong links with health and social care colleagues to ensure effective multi-disciplinary working and develop a strong understanding of local NHS and independent sector service provision in individual's own homes and in care homes.

Greenwich ICB has an exciting opportunity for a Continuing Healthcare Nurse to work with the established Greenwich ICB Continuing Care Team. As a CHC Nurse,you will be responsible for determining the eligibility for NHS Continuing Healthcare funding through new assessments, reviews and appeals in accordance with he National Framework.

Education / Qualifications
Essential criteria
- Registered Nurse – degree level or equivalent
Experience & Abilities
Essential criteria
- Knowledge of National Framework for NHS Continuing healthcare and Nursing Funded Care (Oct 2018 revised) and associated legislation and guidance
- 3 years post registration experience
- Experience of establishing & maintaining good working relationships
- Experience of imparting unwelcome news to patients and their carers for example regarding their prognosis
- Experience of holistic assessment

- Excellent negotiating skills (to promote change & when dealing with resistance to change)
- Ability to assess patients with complex needs
- Understanding of implications of working across the NHS and the independent sector
- Ability to coordinate case conferences/MDT meetings

- Able to manage own workload.
- Self-sufficient in the use of IT – Word , Excel, Microsoft Power Point
