Job overview

These roles which are supported by Macmillan Cancer Support is to work within the cancer team to develop and provide personalised care and support to people who have had a cancer diagnosis ,their family friends and carers, in line with NHS England, NHS Improvement and the Royal Marsden Partners Cancer Alliance personalised care and support interventions The NHS Long Term Plan for Cancer states that “by 2021, where appropriate every person diagnosed with cancer will have access to personalised care, including needs assessment, a care plan and health and wellbeing information and support.

 

Main duties of the job

Working alongside and closely with the Cancer Clinical Nurse Specialists (CNS), this role would be involved in supporting provision of:

 · Personalised Care and Support Planning (based on holistic needs assessments) ensuring people’s physical, practical, emotional and social needs are identified and addressed at the earliest opportunity.

· Health and Wellbeing Information and Support including the provision of accessible information about emotional support, coping with side effects, financial advice, getting back to work and making healthy lifestyle choices. This support will be available before, during and after cancer treatment.

 · End of Treatment Summaries providing both the person and their GP with valuable information, including a detailed summary of treatment completed, potential side effects, signs and symptoms of recurrence and contact details to address any concerns

Detailed job description and main responsibilities

Personalised Care and Support Planning (based on holistic needs assessments, HNA) ensuring people’s physical, practical, emotional and social needs are identified and addressed at the earliest opportunity 
• Participate in the holistic assessment of cancer patients and their family/carers, undertaking electronic HNA’s at designated points in the patient pathway in a structured clinic
 • Develop a care plan, at designated points in the patients’ pathway for the CNS to approve and validate 
• Take appropriate action pertaining to identified needs, be it information and support provision or signposting/referrals to other services 
• Ensure patient and their GP is in receipt of the agreed care plan 
• Practice a good level of communication skills thus empowering the user to make informed choices concerning their care and self-management.
 • Provide confidential wellbeing support to patients/families and carers (telephone and face to face)
• Provide appropriate emotional support to patients/families and carers (face to face and telephone) 
• Utilise effective verbal and non-verbal communication and interpersonal skills with patients, carers and other health care professionals and the general public.
 Health and Wellbeing Information and Support including the provision of accessible information about emotional support, coping with side effects, financial advice, getting back to work and making healthy lifestyle choices. This support will be available before, during and after cancer treatment. 
• Work alongside the cancer team, facilitating health and wellbeing events/workshops for cancer patients and their families/carers 
 • Support, develop and maintain the provision of high-quality cancer information 
• Utilise innovative methods of giving information to ensure equitable access to information including reaching seldom heard groups 
• Signpost/refer patients to the Complementary therapy /Psychology and welfare benefit services 
 • Support patients with provision of headwear for cancer treatment induced alopecia 
• Where appropriate provide accurate promotion/awareness information for patients, carers, staff and the general public

Person specification

Education & Qualifications

Essential criteria
  • 5 GCSEs (Grades A-C) or equivalent qualification including Maths and English
  • ECDL or equivalent demonstrable IT skills
Desirable criteria
  • NVQ 3 or NVQ 2 plus in health or social care
  • Additional training to diploma level or equivalent through short courses.
  • Basic communications training e.g. Sage and Thyme

Experience

Essential criteria
  • Awareness of/Knowledge of relevant cancer treatments, interventions and terminology
  • Experience of working in a health care setting for a minimum of 1 year
  • Experience of working as part of a team
Desirable criteria
  • Knowledge and experience of using NHS IT systems
  • Experience of facilitating groups
  • Experience of coordinating a patient workload
