Due to retirement, we are recruiting to this post as Band 8b Head of Quality for the Learning Disability care Hub. 

This is a 9 month secondment opportunity with a potential for a part time job share opportunity from April 2025. 

It is an opportunity for an experienced practitioner to provide Quality focused leadership to the Learning Disability Care Hub alongside the Senior Leadership Team.  Continuous service development is key to providing the best care and treatment to the service users and their carers we work with, and you will be key in driving this forward.

The care hub is comprised of two residential units, Cambeck Close which offers respite and short breaks to children and adults with a learning disability, and Radcliffe Place which provides a 12 week admission avoidance service for young people aged between 16 and 25 who are experiencing a crisis linked to their learning disability and/or autism. There are five Adult Community Teams for Learning Disabilities and three teams supporting Children with Learning Disabilities. We also have one Autism Assessment Team based in T&G.

The main areas that you will also have leadership responsibility for include:

-             Service user and carer feedback and involvement as Triangle of Care lead for the care hub.

-             Incident and patient safety review coordinator

-             LeDeR – lead on the reporting, reviewing and learning from deaths of people with a learning disability and autism from within the LD care hub.

-             Supporting the Quality aspect of the care hubs business and project plans.

-             Supporting the care hub to achieve the objectives outlined in the GM five year plan to implement improvements and learning linked to health      inequalities for people with a learning disability and autism.

The post holder will be responsible for the delivery of a robust governance framework within a defined locality/specialism as part of the Care Hub Triumvirate leadership team supporting the delivery of a high quality, safe and supportive health services and to support the quality and governance agendas of Pennine Care NHS Foundation Trust.

The post holder will be responsible for co-ordinating all quality and governance activity including the development and / or implementation of policies, risk management strategies, training programmes and audit activity. 

The post holder will be responsible for the planning, delivery and review of a comprehensive range of mental health services

The post holder will participate in the on call rota.

Care Hub Triumvirate
• To be a core member of the care hub tier 3 triumvirate working alongside the care hub associate director and clinical director to ensure effective planning, delivery and review of a comprehensive range of mental health services within the care hub.
• To hold joint responsibility for the management of the care hub ensuring the effective implementation of both national and local strategies and the 
achievement of service objectives within available resources.
• To represent the care hub at a senior level in both internal and external 
meetings.
• To receive escalations for the care hub in relation to patient safety, 
experience and effectiveness and to ensure appropriate management of 
these to mitigate/reduce any identified risk.
• A requirement to support teams to understand their responsibility and 
accountability for safety, experience and effectiveness. Including, undertake difficult conversations with staff and/or teams to hold them to account when concerns are raised in relation to clinical practice impacting on patient safety and quality of care.
• To consider and escalate any concerns regarding practice or safety that may require referral to registered bodies (ie NMC, Social Work England, HCPC etc).
• To consider and escalate any concerns regarding practice or safety that may require the input of local authorities (ie safeguarding, management of allegations etc).
• To represent or deputise, where requested, in the absence of the Network Director Quality, Nursing and AHPs
• To provide cover for the Associate Director of the Care Hub in their absence

Patient Safety
• To embed clinical governance, safety and quality into all aspects of the Boroughs mental health services
• To review and oversee the management of incidents reported across the Care hub liaising with local services and the central risk management team to ensure appropriate review is completed, lessons learned are identified, actions have been taken at a local level and appropriate attention is given to those incidents meeting the criteria of a serious incident.
• To organise and facilitate the robust investigation of serious and untoward incidents in accordance with Trust policies and procedures, and ensuring that appropriate local review and sign off procedures are in place prior to submission to the Trust Patient Safety Improvement Group. 
• To work in line with current and developing national frameworks. For example Patient Safety Investigation Response Framework. 
• To be an active representative of the Care Hub or Borough at the network quality and safety panel and network quality assurance group presenting serious incident investigations completed within the Borough for Trust sign off, and contributing to the scrutiny of other serious incident investigations.
• To embed clinical governance, safety and quality into all aspects of the care hubs mental health services
• To develop and manage risk and incident reporting links with the Integrated care board and/or Lead Provider Collaborative (LPC) to ensure information is supplied accordingly.
• To support investigation leads within the Care Hub or Borough to complete robust case assessment and investigation as required.
• To lead the investigation of serious incidents using appropriate methodology within the Care Hub or Borough and across the organisation as appropriate. This will include speaking to patients and family members, to gather highly complex, sensitive information that will inform the investigation. 
• To facilitate desktop review and action reviews within the Care Hub or Borough and across the Trust as required.
• To monitor trend and themes in incident reporting, incident types and lessons learned, identifying any areas for actions and working with local clinical leads to formulate measures required to address any patterns identified, e.g. deep dive, cluster investigations etc.
• Lead on infection prevention and control, supporting the Trust’s DIPC in their objectives and liaising where appropriate with the clinical lead for IPC
• Ensure robust arrangements are in place for safeguarding Adults and Childrenand liaising where appropriate to the Head of Safeguarding
• Lead on Physical health care for the Care Hub or Borough liaising where 
appropriate with the Modern Matron – Physical Health and Head of Nursing.
• Lead on End of Life care for the Care Hub or Borough liaising where appropriate with the Head of Nursing
• Oversight on safe staffing and actions to address gaps, including monitoring dependency and fill rates in services
• To regularly provide advice, support and oversight to complex and high risk cases from a quality and safety lens
• To lead or coordinate action plans or improvements and involve other subject matter experts (where appropriate) for complex situations or incidents.
• To ensure that any learning is embedded within the Care Hub and shared within or outside of Trust as appropriate
• To undertake investigations that may be complicated, contradictory and highly sensitive with conflicting information received from a range of sources including bereaved families, safeguarding and the criminal justice systems. Although certain aspects of information may be readily available consideration has to be given to human and unknown factors which may lead to ambiguous outcomes for the receiving audience.
• Where appropriate lead on the reporting and sharing of information with the Lead Provider Collaborative around quality and safety. 
• Where appropriate, represent the Care Hub at meetings and liaison with the Lead Provider Collaborative
