Job overview

Service Summary:

The Home Treatment Team will provide intensive support for patients as an alternative to hospital admission. 

 

The Home Treatment Team hours of operation are 24 hours 7 days a week.  thus ensuring the service user can be appropriately referred or signposted to existing services the following morning if required. Research suggests that currently there is a peak of presentations of individuals to the emergency department in the evening which is more likely to lead to admission  It has been recognised that most admissions happen in the evening and when service users present in crisis it is usually in the evening. 

It is envisaged that this approach will provide a timely and quality experience for the service user and avoid an inpatient admission where clinically appropriate.

 

Main duties of the job

Key Responsibilities of the Post:

The post holder will work as part of the  Home Treatment teams to deliver an effective and efficient response to people who access the service, providing clinical assessments and interventions focusing on relapse prevention and improved quality of life for service users. 

Key Requirements of the Person Specification:

  • RMN / Occupational Therapist / Social Worker equivalent.
  • Completion of Preparation for Mentorship/Practice Teacher/Fieldwork Educator qualification or willingness to complete
  • Short courses and experience to post graduate diploma level
  • Post registration experience.
  • Proven experience of working with clients with complex needs

Detailed job description and main responsibilities

Clinical 
 To provide robust gatekeeping service for all requested inpatient admissions for 
the area with the exception of A&E which will be gate kept by liaison mental health service. 
 To carry out a thorough assessment of the service users’ current mental health 
problems and individual needs, including a risk assessment.
 To develop individualised packages of care for service users with mental health problems, including signposting where necessary, within the stepped care model (NICE, 2003), Fair Access to Care Services and the Care Programme Approach.
 To provide intensive home treatment in service users home / community settings.
 To provide crisis management and short term psychotherapeutic interventions as appropriate
 To develop and provide a supported discharge / leave pathway for inpatients
To identify the need for protection in line with safeguarding children and the Protection of Vulnerable Adult Policy and to comply with statutory obligations.
 To liaise and negotiate with other professions, statutory and independent agencies to ensure the best possible service is provided to service users and carers.
 To evaluate systematically and record, outcomes of mental health assessments, adapting care on the basis of these outcomes.
 To involve family and carers in the assessment and care planning with service users consent.
 To provide education and appropriate self-help resources to patients and carers specific to individual needs.
 To maintain accurate and up to date clinical documentation on all service user contacts, ensuring confidentiality at all times.
 To develop a thorough understanding of deliberate self-harm, suicide, risk formulation and possible management strategies.
 To contribute to the establishment and review of policies, procedures and practice guidelines relating to the work of the Home Treatment Service and the interface between primary, secondary and specialist Mental Health Services.
 To undertake evidence based practice.
 To observe and practice within the legal requirements of the Mental Health Act 1983.
 To maintain professional awareness by keeping abreast of changing trends in clinical / professional practice.
 To be aware of professional accountability, with due regard your professional Code of Conduct.


Research and Audit
 To ensure that PARIS records are efficiently maintained in a timely manner.
 To contribute to audits as required, providing relevant data as required in line with performance monitoring targets.
 To record, monitor and inform of any unmet need.
 To participate in research opportunities as and when appropriate.


Education and Training
 To contribute to the ongoing training and development of non-mental health trained medical and nursing staff with regard to mental health issues.
 To have effective presentation skills in order to positively promote the Home Treatment Service.
 To participate in clinical and managerial supervision in accordance with Pennine Care NHS Foundation Trust policy.
 To provide mentorship, education and training to learners from a range of professional backgrounds. 


Service Development
 To participate in monitoring and evaluation of the effectiveness of service delivery and contribute to developments relevant to mental health services.
 To take an active role in Working Groups and Service development initiatives when requested.
 To participate in the rota planning for the Service and ensure that the Team Manager is informed of any gaps in service provision.


Management and Personnel
 To provide clinical and peer supervision, advice and support to all team 
members.
 To provide supervision for junior members of the team and facilitate the 
completion of their IPDR’s
 To deal appropriately with clinical incidents, complaints etc from patients, relatives and members of the public, via line manager, in accordance with Trust policy

Safeguarding
 Appointments to regulated and controlled activities require an enhanced DBS disclosure. 
 All staff have a responsibility to promote the welfare of any child, young person or vulnerable adult they come into contact with and in cases where there are safeguarding concerns, to act upon them and protect the individual from harm.
 All staff should refer any safeguarding issues to their manager and escalate accordingly in line with the Trust Child and Adult Safeguarding Policies.
 All staff should familiarise themselves with the NICE Guidelines “when to suspect child maltreatment 2009.” 


Professional and Personal Development 
 The post holder must ensure that they are aware of their responsibilities by attending the Trust Mandatory Training and Induction Programme.
 The post holder will be involved in a formal IPDR/KSF review with their manager at least every 12 months. Once performance / training objectives have been set, the staff member’s progress will be reviewed on a regular basis, so that new objectives can be agreed and set, in order to maintain progress in the service delivery.
 The post holder will be expected to take responsibility for their own professional development and will be supported by the Trust to achieve development opportunities as appropriate.


Confidentiality and Information Governance
 Confidentiality is of prime importance. In the normal course of duties, the post holder will have access to confidential documents and information relating to patients, service users, staff and contractors, as well as information of a 
commercially sensitive nature. Such information should not be communicated to anyone outside or inside the NHS unless done in the normal course of carrying out the duties of the post. Disciplinary action will be considered where a breach of confidence has been established.
 All information obtained or held during the post-holders period of employment that relates to the business of the Trust and its service users and employees will remain the property of the Trust. Information may be subject to disclosure under legislation at the Trust’s discretion and in line with national rules on exemption.
 The post holder must maintain high standards of quality in corporate and clinical record keeping ensuring information is always recorded accurately, appropriately and kept up to date. The post holder must only access information, whether paper, electronic or in other media, which is authorised to them as part of their duties.
 The post holder must work to the requirements of data protection laws as applicable to the UK, which includes the General Data Protection Regulations (GDPR).

Education/Qualifications

Essential criteria
  • RMN / Occupational Therapist / Social Worker equivalent.
  • Completion of Preparation for Mentorship / Practice Teacher / Fieldwork Educator qualification or willingness to complete
  • Short courses and experience to post graduate diploma level
  • Evidence of on-going CPD
Desirable criteria
  • Non-medical prescribing qualification
  • Formal teaching qualification.
  • Further training in brief psychosocial interventions.

Experience

Essential criteria
  • Post registration experience.
  • Proven experience of working with clients with complex needs.
  • Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness
  • Experience of working in multi-disciplinary teams.
  • Experience of providing supervision to junior staff
Desirable criteria
  • Adult acute mental health work experience
  • Experienced in the use of outcome measures such as CORE, PHQ9 and GAD7
  • Experience of working with a recovery model of care

Knowledge
