Job overview

An exciting opportunity has arisen for a Senior Practitioner to work within the South Mersey Community Mental Health Team.   You will play a crucial role in the clinical leadership of the team and support staff to provide a high-quality service to service users and carers. We are looking for an experienced and enthusiastic practitioner who has a real passion for providing support to promote mental and physical heath, wellbeing and recovery.  


Your role will involve supporting the manager in all aspects of managing the services as well as provide clinical leadership and patient care within the team.


You will be expected to support other practitioners within the service in development of their skills by offering highly developed risk assessments and management plans. 


Leading with a positive and helpful attitude is essential for the team, so you will be an excellent communicator and demonstrate the ability to motivate the team to deliver high quality and responsive care.

The successful applicants will be able to demonstrate both their clinical abilities and leadership qualities, whilst promoting high quality services. 

The post holder will be expected to participate in the service bronze on call and bronze on site rota as required. 

Main duties of the job

The post holder will act as a care coordinator for a small number of highly complex cases in line with the Trust Care Programme Approach (CPA) guidance and policies. 

To be responsible for the day to day clinical leadership and development of a designated multidisciplinary team which is integrated, flexible, user-focussed and recovery-orientated.

To deliver a safe, effective, and high quality service to service users with mental health needs, to ensure delivery to the relevant clinical pathways, as outlined within the CMHT operational procedure.

Participation as a member of the Community Services Management Team in the overall management of community based secondary mental health services.

The post holder will aim at all times to promote the independence and well-being of adults, to assist social inclusion and valued community presence and to enable the widest possible expression of chosen lifestyle.

Education/Qualifications

Essential criteria
  • Registration to professional body: OT, SW England NMC.
  • Previous experience working within MDT setting in MH
  • Experience of complex mental health
Desirable criteria
  • Previous managerial experience
  • Previous experience supervisory role
  • Previous experience within CMHT
  • Previous experience complaints and governance

Experience

Essential criteria
  • Extensive experience of working with adults of working age
  • Experience of working with people with complex needs and co-morbidities
  • Experience of working in a recovery focussed way
  • MDT Working
  • Experience of conducting assessments identifying health and social care needs.
Desirable criteria
  • Working within rehabilitation services
  • Experience of supporting people who misuse alcohol or drugs
  • Experience of delivering line management and clinical supervision

Knowledge

Essential criteria
  • Working knowledge and understanding of CPA
  • Knowledge and understanding of recovery model, appropriate therapeutic approaches and engagement strategies
  • Knowledge and understanding of Mental Health Act, Mental Capacity Act & Care Act

Skills

Essential criteria
  • Be able to manage a caseload and work autonomously within the team
  • Ability to act as a care coordinator under the CPA framework
  • Ability to assess, plan and evaluate care
  • Have evidence of good time management and organisational skills being able to work under pressure.
Desirable criteria
  • To be able to plan and deliver recovery based activities within service users own homes and local communities.
