Department Summary:
Tameside West CMHT is the Community Mental Health Team which supports working age adults across Denton, Droylsden, Audenshaw and Dukinfield.. The team is comprised of community psychiatric nurses, social workers, community support workers, carers' support worker, administration staff, and team clinical leadership and team management. The team is also based within a wider Multi-Disciplinary Team, with daily access to Psychiatric Consultants, Psychologists, Occupational Therapists, Mental Health Wellbeing Practitioners, amongst other professions. We also work in partnership with social workers and support workers from Tameside Adult Social Care.
Role Summary:
To undertake the role of Care Co-ordinator in the assessment, treatment, and delivery of care and support to individuals living with a wide array of severe, enduring and complex mental health needs, who all meet the criteria for secondary care NHS mental health services.
You will ensure that the highest quality care is carried out in accordance with the policies and procedures of Pennine Care NHS Foundation Trust and relevant, regulatory bodies.
Main duties of the job
To undertake holistic mental health and risk assessments using trust approved documentation and procedures.
To manage a caseload of clients who will have a variety of mental health conditions, both organic and functional.
To provide evidence based therapeutic interventions promoting enablement, choice, and recovery.
To comply with all legislative requirements in relation to service users care.
The post holder will support the duty system that is operated within the team and respond to crisis calls they receive.
Actively participate in multi-disciplinary meetings and contribute to clinical decision making.
Engage and supervise student nurses.
This role does not involve commissioning services on behalf of the Local Authority, however we can support our Adult Social Care partners with this process, by providing health-based assessments of mental health needs. We can have a role in referring for, and reviewing, funding for services funded by Greater Manager ICB, in alignment with Section 117 of the Mental Health Act.

Education / Qualifications
Essential criteria
- RMN/Social Worker/OT
- Registered with NMC or SW England or HCPC
- Completion of appropriate Preceptorship
- Substantial post registration experience and/or short courses to post graduate diploma level
Desirable criteria
- Mentorship qualification
- Qualification in psychologic therapeutic interventions
Experience
Essential criteria
- Substantial experience of relevant post registration practice
- Experience of managing a caseload
- Experience of working in a multi-disciplinary team setting
- Experience of working with people with a range of severe and complex mental health problems
Desirable criteria
- Experience of working within a Community setting
- Experience of clinical supervision of other staff
Knowledge
Essential criteria
- Good working knowledge of therapeutic assessment techniques and interventions in the management of behaviour of individuals with complex mental health needs
- Knowledge of a range of screening and assessment tools and interventions utilised to support individuals with severe and complex mental health problems
- Comprehensive understanding of Safeguarding adults procedures and practice
- Comprehensive understanding of the Mental Capacity Act, Mental Health Act and Care Act
- Understanding of key national and local policies and drivers in relation to adult mental health services.
Desirable criteria
- Knowledge of Local Authority and CCG commissioning processes
Skills and Abilities
Essential criteria
- Ability to undertake the key worker role
- Evidence of ability to provide specialist mental health assessment and risk assessment / management and be accountable for decision making
- Excellent written and interpersonal communication skills
- Ability to manage own emotions and deal with others in highly stressful situations
- Ability to engage with and provide therapeutic interventions for this client group and carers
- Ability to prepare, record and complete relevant care records and reports to a professional standard
- Evidence of ability to manage a diverse and complex caseload, prioritise own time and work under pressure
- Ability to coordinate and delegate the work of junior staff
- Ability to value diversity and respond constructively to discriminatory behaviour
- IT Skills
