Job overview
To work within the Learning Disability Directorate as member of the Children’s Learning Disability Team which currently sits within the Tameside and Glossop Learning Disability Team.
The post holder will provide specialist health and behaviour assessments to children and young people with a learning disability who are referred in to the team.
Interventions will be delivered across different settings including the person’s home. There will be a requirement to liaise with and work collaboratively with partners and stakeholders to ensure responsive, effective, safe and child centred services are delivered.
Main duties of the job
• To work in accordance with Pennine Care NHS Foundation Trust policies and procedures at all times.
• To provide a safe, effective, responsive, efficient and timely community nursing services to children and young people with learning disabilities, their families and carers.
• To undertake a range of comprehensive assessments for children and young people with a learning disability and complex and challenging needs. This assessment work requires a specialist knowledge base on which to practice.
• • The post holder may be required to undertake additional training in implementing high quality Positive Behavioural Support
• The post holder will be expected to offer behaviour support training to identified others including teams around the child
• To develop a range of interventions and specialised care plans.
•
• To embrace the public health role, promoting health and wellbeing and where possible, prevent ill health.
• To support the work of the Community learning disability nurses, in reducing the health inequalities faced by people with learning disabilities.
• To work collaboratively with primary care and secondary care services to ensure the health needs of people with learning disabilities are met.
• The post holder will undertake an active role in identifying and managing risk.
• The post holder will be required to work in close partnership with Social Services and Child Safeguarding.

Education / Qualifications
Essential criteria
- Registered Nurses Learning Disabilities (RNLD)
- Post registration training / willingness to undertake training plus experience to postgraduate diploma level.
Desirable criteria
- Training in Positive behavioural approaches
- Training in Clinical Supervision/Mentors hip
Experience
Essential criteria
- Demonstrate knowledge and experience in supporting individuals who have a learning disability
- Experienced in delivering advice and support to a wide range of people.
- Experienced in partnership working
- Experienced in managing risk.
Desirable criteria
- Experienced in delivering training.
- Previous community nursing experience desirable
- Experience in supporting individuals with complex health needs
Knowledge
Essential criteria
- Person Centred approaches.
- Knowledge of using LD specialist assessment tools
- Knowledge of behavioural techniques /managemen
- Ability to deal with complex sensitive and distressing situations which may include challenging behaviour, abuse and breaking bad news
- Knowledge and skills across a range of learning disability issues
- Knowledge of Child Safeguarding and Adult Safeguarding
- Knowledge of current strategic drivers for community learning disability teams
Desirable criteria
- Knowledge of health issues that impact upon children and young people with a learning disability
- Knowledge of Transforming Care
- Knowledge of SEND and EHC
Skills and Abilities
Essential criteria
- Ability to complete complex assessments
- Highly developed communication skills to support a range of activities
- Ability to collaborate in the completion of formulations.
- Ability to develop multi-element interventions / support packages.
- Ability to incorporate key principles of ‘Valuing People’ into direct client care.
- Ability to proactively manage competing demands
- Experience in supporting people with complex and challenging needs
