Cystic Fibrosis Social Worker

An exciting opportunity has arisen for a Band 6 Social Worker to join the Manchester Adult Cystic Fibrosis (CF) Team, located at Wythenshawe Hospital, and part of the Manchester University NHS Foundation Trust.  

The Manchester Adult Cystic Fibrosis Centre (MACFC) is a specialised regional centre that provides personalised and holistic care and support to over 480 individuals with CF who live in the Northwest.

You will be a core member of the CF Multidisciplinary Team (MDT), collaborating closely with clinical psychologists, specialist nurses, dietitians, physiotherapists, occupational therapists, and medical staff.

 We are looking for someone self-motivated, dynamic and passionate about delivering excellent psychosocial care to adults with CF.  You will need to be flexible, a team player, and able to demonstrate a commitment to professional development and evidence-based practice. 

You will provide proactive social work support to individuals with CF, and their families where necessary, around a range of psychosocial issues, including access to resources, community care, employment or education, housing, end of life care and bereavement, as well as supporting those with additional vulnerabilities such as learning disability, mental health, substance misuse or other issues.

The successful candidate will work 25 hours over 3 days with the aim of: 

  • Providing high-quality social work support to individuals and families in inpatient, outpatient, and community settings.
  • Assessing the psychosocial conditions of individuals with CF and their families and connecting them with resources to meet their needs.
  •  Offering professional social work guidance on social work and psychosocial issues to the wider CF Team.

  • Registration with a Social Work England

  • Post-qualifying qualification: e.g. Practice Educator/Best Interests Assessor/Approved Mental Health qualification

Experience

  • Post-qualifying experience in assessing vulnerable individuals using personalised care principles to identify their needs, strengths, challenges, and risk areas, and co-producing support plans to meet those needs.
  • Managing complex cases and involvement in multi-agency work.

  • Working with individuals with a life-limiting condition.

Knowledge

  • Key theories, methods, research, policies and legislation relating to social work with adults.
  • Day-to-day and longer-term psychosocial impacts of living with chronic and progressive health conditions on the lives of individuals and their families/carers

  • Awareness of CF Standards for the clinical care of children and adults with CF in the UK.

Skills and abilities

  • Strong interpersonal communicaton skills - using different ways of communicating
  • Organisational skills - ability to organise and prioritise work to maintain a balanced workload, time management, efficient record-keeping.
  • Use of critical reflections to evaluate practice, decisions and actions, and identify areas for improvement.
