Job overview
Haemoglobinopathy Nurse Counsellor-Band 6 ,to join a team ,that provide support for patients with Sickle Cell Disorder. Providing community support ,attending Consultant clinics for patients discharged from the Sickle Cell Unit/Ward 10 ,MRI. Supporting patients in the home following discharge.
Main duties of the job
Haemoglobinopathy Nurse Counsellor will be part of community nurses working with SCU at MRI. Attend sickle clinic for patients discharged from Ward 10/ SCU, support and follow up with community visits and specialist adult nursing in community. Provide health awareness and self care management.
Establish an education and health support facility for adults affected and families/carers and provide continued care and education, support and advocacy.
