Job overview

The Patient Safety Learning Response Lead (Insight and Involvement) is a central role in the Trust’s implementation of the Patient Safety Incident Response Framework (PSIRF), and supports multiple elements of our safety management system, including supporting patient safety oversight and insight, analysing and integrating multiple sources of patient safety intelligence, and supporting the recruitment, support and development of Patient Safety Partners across the Trust.

The Patient Safety Learning Response Lead (Insight and Involvement), as part of the Learning Response Lead team, will be responsible for leading and coordinating all Patient Safety Incident Investigations (PISIIs) across the Trust in line with the expectations of PSIRF, engaging with a range of stakeholders to identify opportunities for learning using recognised tools encompassing principles of human factors and ergonomics, systems engineering, psychology, and investigation best practice as required. A high level of sensitive and compassionate engagement with patients, families, staff, and others affected by incidents is expected of this role. 

Main duties of the job

Responsible for management of the Trust Safety Oversight System, ensuring intelligence including horizon scanning, active surveillance and other data is integrated and presented effectively. Analysing data from multiple sources, understanding complex interactions and ensuring robust conclusions are drawn and shared with a range of stakeholders with different levels of understanding

Responsible for supporting areas of patient safety profiling, where risk has been identified or in support of Trust-wide priorities. 

Using in depth knowledge of patient safety data analysis, including an understanding of human factors approaches, to support the development of a range of system reliability measures to support effective reporting and escalation of emergent safety concerns.

Use highly specialist knowledge to be professionally responsible and accountable for own caseload of concurrent systems-focussed, high quality, Patient Safety Incident Investigations (PSIIs) in line with national guidance incorporating best practice identified as part of the implementation of PSIRF nationally, working autonomously in leading the delivery of timely and effective investigation reports

 Working with the Patient Involvement Team to inform the design and implementation of a network of Patient Safety Partner roles across the Trust, including  access to relevant training and support, links into  peer networks and the network of staff Patient Safety Specialists at MFT

Person specification

Qualifications

Essential criteria
  • Clinical background or evidence of a substantial understanding of safety management and healthcare/ clinical issues
  • Educated to Masters level OR equivalent level of experience of working at a senior level in specialist area (e.g., healthcare, patient safety, human factors, incident investigation, industrial safety management)
  • Evidence of continuing professional development.
  • Have completed the national Patient Safety Syllabus levels 1&2
  • Completion of relevant patient safety incident investigation training (e.g. Healthcare Safety Investigation Branch Level 2/3 patient safety investigation training or equivalent).
  • Recent training on systems approaches to learning from patient safety incidents
  • Evidence of recent relevant continuous professional development
Desirable criteria
  • Evidence of undertaking or have completed level 3 and/or 4 of the national Patient Safety Syllabus

Experience

Essential criteria
  • Extensive experience of working within an NHS or similar health care organisation patient safety or safety management/ improvement environment
  • Experience of completion of effective safety investigations that meet the patient safety incident response standards set out in the national Patient Safety Incident Response Framework for England
  • Experience of using human factors and systems-based approaches to understanding and analysing patient safety improvement opportunities
  • Experience working with multidisciplinary teams at senior level
  • Experience of working within statutory guidance and requirements.
  • Experience in engaging compassionately and effectively with those affected by safety incidents and supporting a just and restorative incident response
Desirable criteria
  • Experience of working at a senior level in the NHS
  • Working experience of Ulysses
  • Working experience of using electronic patient records to develop chronologies
