Job overview

An exciting opportunity has arisen within our Continuing Health Care Teams across Greater Manchester.  We are looking for highly motivated and experienced band 5 RMNs, RGNs or Allied Health Care professional  who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined.

The successful post holders will have the opportunity to work within a successful, progressive Integrated Care organisation and make a positive contribution to the lives of people in Greater Manchester. 

Working within the remit of the National Framework for Continuing Healthcare, the post holders will deliver an effective and competent level of service, consistently delivering a client-focused service.

The post holders will assist and support in the facilitation of patient reviews in line with standard operating policy and procedures, ensuring the National Framework for Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes.

 

 

Main duties of the job

  • Working within a team ensuring a high-quality service is delivered
  • Managing a caseload of CHC eligible individuals.
  • Facilitating and contributing to multi-disciplinary assessments and patient reviews in line with the National Framework.
  • Ensuring the principles and duties of safeguarding are holistically, consistently and conscientiously applied.
  • Supporting the Continuing Healthcare team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery.
  • Ensuring that all processes that are implemented are compliant with the National Framework
  • Commitment to completing all mandatory training.
  • Assisting the for Continuing Healthcare with resolution of complaints, appeals and disputes as well as capturing of lessons learned to mitigate these instances.
  • Supporting delivery and development of standard operating procedures and processes, including those for Personal Health Budgets (PHBs), within the team.
  • Maintaining professional registration and up-to date knowledge of legislation as well as demonstrating accountability for own actions according to relevant professional code of conduct.
  • Delivering against objectives, achieving quality outcomes.
  • Prioritising own workload and working to tight deadlines.
  • Ensuring a person-centred approach with individuals, families and representatives.

The role sits in the Continuing Healthcare & Individualised Commissioning team which is part of a wider Directorate led by the Associate Director of Nursing, Quality and Safeguarding.

KEY DUTIES AND RESPONSIBILITIES

 

• Working within a team ensuring a high-quality service is delivered

• Supporting the Continuing Healthcare team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery. • Supporting the Practitioner in ensuring that all processes that are implemented are compliant with the National Framework  

• Commitment to completing all mandatory training.  

• Deputising as required in the absence of the Practitioner.

• Assisting the Practitioner for Continuing Healthcare with resolution of complaints, appeals and disputes as well as capturing of lessons learned to mitigate these instances.

• Supporting delivery and development of standard operating procedures and processes, including those for Personal Health Budgets (PHBs), within the team.

  • Nursing qualification

Qualifications

  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level
  • Evidence of Continuing Professional Development

Experience

Essential criteria
  • Experience and understanding of evaluating and measuring the performance of health services
  • Experience of working effectively with clinicians, health and social care colleagues and other partner agencies
  • Previous experience of working within a busy environment with ever changing priorities

Knowledge

Essential criteria
  • Knowledge of specialist areas acquired through experience and training plus further knowledge of the Continuing Healthcare process
  • Knowledge, understanding of the Personal Health Budget process
  • Negotiation and conflict management skills and the ability to influence
  • Knowledge of Data Protection

Competencies

Essential criteria
  • Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales
