Service/Ward/Department Summary:

An exciting opportunity has arisen for a suitably qualified and motivated person to join our Community Mental Health Team. This new role will have particular responsibility for assessment and intervention into residential nursing homes.

Role Summary:

The post holder will have extensive experience of working with older people with dementia, preferably in both in-patient and community settings. You will have excellent assessment and communication skills and the ability to work flexibly according to service need.

Main duties of the job

Key Responsibilities of the Post:

  • To offer rapid and skilled assessments to residential and nursing homes, enabling them to meet the needs of residents presenting with complex and challenging problems
  • To develop care plans within these settings, Prioritising non-pharmacological interventions the aim of preventing unnecessary hospitalisation or moves to higher levels of care
  • To manage the work of support workers within the service
  • To build supportive relationships with colleagues across the care sector, working with sensitivity and tact

As part of the wider Older Peoples Mental Health Services an opportunity has arisen for a Mental Health Practitioner to join the Care Home Liaison Team within the Older Peoples Community Mental Health Liaison Service.

 

This award winning team is based at The Meadows within the borough of Stockport.  The service provides a resource into a variety of settings to support a range of disciplines from health, social care and the third sector in order to meet the mental health needs of our service users.

Detailed job description and main responsibilities

Assessment of residents who are experiencing Behavioural &Psychological symptoms (BPSD) of dementia.

To be able to assess residents presenting with co-morbidities of physical and mental health needs including the impact of medication and infection.

Formulate plans of care and manage the work of support staff in delivering care which may involve modelling care techniques to care home staff.

Building and developing relationships with care providers in residential and nursing settings.

To identify the need for protection in line with the Safeguarding Adults policy and comply with local procedures and relevant training.

To have a thorough understanding of functional and organic illness in older people and management strategies to support residents in a care home setting.

To involve carers and relatives in care planning and decisions related to patient care.

Person specification

Education / Qualifications

Essential criteria
  • Registered Mental Health Nurse, Occupational Therapist or Social Worker.
  • Evidence of continuing professional development to post graduate diploma level.
  • Mentor qualification or willingness to complete
Desirable criteria
  • Degree
  • NMP qualification

Experience

Essential criteria
  • Experience of working within Older Age Services
  • Good working knowledge of physical health problems in the older person and assessment skills in this area
  • Knowledge of medication used in this service user group
  • Ability to monitor effects and side effects
Desirable criteria
  • Experience of community working
  • Skills training in physical health assessment
  • Experience of delivering training

Knowledge

Essential criteria
  • Thorough understanding of dementia and the impact this has on care providers as well as the individual.
  • Understand of the impact of physical health problems for this group of service users, and a knowledge of delirium.
  • Awareness of relevant mental health legislation i.e. MHA/MCA/DoLS
  • Experience of MDT working
  • Proven experience in delivering non-pharmacological interventions in dementia with the aim of reducing medication use.
Desirable criteria
  • Evidence of management skills/previous supervisory experience
  • Awareness of End of Life issues in dementia care
