We have an exciting opportunity for a qualified practitioner (Nurse/ OT/ Social Worker/ Mental Health Wellbeing Practitioner) to work within the Neighbourhood Mental Health Team in Heywood, Middleton and Rochdale.
This is a 6‑month fixed-term contract within the team, working across GP surgeries to support individuals aged 18 and over with severe and enduring mental health conditions, as well as those presenting with trauma and complex needs.
The post holder will play a key role in providing advice and triage, carrying out clinical assessments where appropriate, and offering short-term interventions for individuals experiencing complex mental health difficulties. Where required, you will facilitate referrals and transitions into the most suitable services within the transformed Community Mental Health Service.
In this role, you will be part of an experienced, knowledgeable, hardworking, and highly supportive multidisciplinary team.

We currently have a vacancy for a full time Band 5 Community Mental Health Practitioner within our HMR Neighborhood Mental Health Team.
Please see JD for full information- please note this refers to Care coordination but this role is working within NMHT. We are a service that works with adult’s age 18 years and upwards.
This is a full time post working core hours of Monday to Friday, condensed hours over 4 days will be considered.
PARIS training would be an advantage however training will be provided if required.
You will be working as part of a very experienced team and this opportunity would make a good pathway for anyone wishing to pursue or develop a career in community mental health.

- At degree level / equivalent experience.
- Willingness to undertake further training relevant to the practitioner role resulting from legislative or service changes, as appropriate.

- Mentorship in Healthcare Practice (MSLAP
Experience

- To have worked in a mental health inpatient setting as a student or post qualified.
- Experience of working with clients presenting with co-morbidity.

- Post qualifying experience in a relevant area of mental health practice.

- Ability to demonstrate effective time management and disciplined self - organisation.
- Ability to provide advice to Assistant Practitioners.
- Ability to provide appropriate education and training to peers and assistant as appropriate.
