Due to investment, we are looking for an enthusiastic and motivated pharmacist to join our pharmacy team, supporting Outpatient Parenteral Antimicrobial Therapy (OPAT) and Hospital @ Home within Salford. 

As a senior pharmacist you will work alongside consultant microbiologists, acute physicians, specialist nurses and the wider multidisciplinary team to ensure safe, effective, patient-centred therapy.

You will join the trustwide Antimicrobial Pharmacy Team to support antimicrobial stewardship (AMS) activities / workstreams within relevant areas.

You will also work alongside our existing Hospital @ Home pharmacist to oversee and coordinate the H@H pharmacy clinical service, encompassing new ways of working, providing pharmaceutical care to respiratory, cardiology and frailty patients.

You will be responsible for developing pathways and guidelines, ensuring seamless care across primary and secondary settings.

You will need to have excellent communication skills, the ability to work within a multidisciplinary team and a passion for clinical pharmacy.

The successful candidate will be provided with support from an experienced pharmacy team, as well as opportunities for personal growth and career development.

Main duties of the job

Key responsibilities will include

·       Service development and delivery, in line with national and local guidance

·       Participating in MDT and Consultant Ward rounds

·       Clinical pharmacy reviews of patients, virtually or face to face within the home environment, utilising your independent prescribing qualification as necessary

·       Ensuring safe practice and development of guidelines and procedures

·       Providing leadership on the use of medicines

·       Education and training of staff on safe and appropriate use and prescribing of medicines

  • Work with key stakeholders across the interface of care to support with medicines related initiatives
  • Support the delivery of other community medicines optimisations workstreams when required

 You will need access to a car for work purposes and a clean driver’s licence, as the role involves travel between bases and home visits.

Qualifications

Essential criteria
  • Vocational Master’s degree in Pharmacy or equivalent

Registration

Essential criteria
  • Registration with the General Pharmaceutical Council of GB as a practicing pharmacist
  • Post Graduate Diploma in Clinical Pharmacy or equivalent (e.g. CPPE Primary Care Pathway)
Desirable criteria
  • Non-medical prescriber or undertaking the non-medical prescriber course (If not yet qualified as a non-medical prescriber, you will be expected and supported to obtain this qualification in order to support practice in this role)

Knowledge, training, experience

Essential criteria
  • Good all-round clinical knowledge and practical clinical skills
  • Experience of hospital pharmacy services/primary care
Desirable criteria
  • Experience of service development

Skills and abilities

Essential criteria
  • Ability to work on own initiative, prioritise work and lead a team
  • Excellent communication skills, both verbal and written
  • Skills for analysing prescriptions, clinical systems and complex patient, medicines or financial information in order to provide advice on medicines, dosages and expenditure.
