An opportunity has arisen within Greater Manchester Integrated Care Board (GMICB), Stockport Locality's Continuing & Complex Healthcare Team, for a Nurse Assessor (RGN/RMN)

 

Main duties of the job

GMICB, Stockport Locality, is seeking to recruit a highly motivated, enthusiastic and forward thinking nurse to work as part of a professional team. The successful candidate will need to be able to demonstrate knowledge and skills in person-centred assessments, and the ability to work within a multi-agency environment.

The successful applicant will be required to work autonomously and collaboratively within a dynamic skill-mixed team. As a Nurse Assessor your role will include assessing individuals against the National Framework for NHS Continuing Healthcare and Funded Nursing Care, undertaking complex case management and annual reviews.

If you are a self-motivated and collaborative professional, who possesses excellent communication skills supporting the philosophy for delivering high quality services to people, this would be the ideal opportunity for you.

Previous experience within a Continuing Healthcare Team would be an advantage, but training and support is available for the right candidate.

Detailed job description and main responsibilities

Promote GMICB'S vision, and mission and to uphold GMICB’s values in all day-to-day activities and delivery of services; participate in the objective setting process as part of the annual Performance Development Review/appraisal process, to understand how own role and objectives are linked to team, directorate and corporate objectives, to review what aspects of your role are being done well, and to identify any areas for development. 

To provide comprehensive clinical advice, assessment and case management to NHS Continuing Health Care and NHS Funded Nursing Care functions for GMICB, with particular responsibilities for people who have complex health care needs, this includes all categories of needs eg physical, end of life,  Mental Health and Learning Disability needs.

  • NMC registered
  • Recent and up to date professional development
Desirable criteria
  • Training in continuing Healthcare assesments and processes

Experience

Essential criteria
  • Experience working in a multi disciplinary team setting
  • Nursing experience in a health environment
  • Experience of working with service users with complex and enduring health needs
Desirable criteria
  • Experience of contributing to the continuing Healthcare assesment process
  • Experience of working in a Continuing Healthcare team
  • Experience as a first line manager and evidence of management training
  • Experience of working with people with Mental Health and complex needs

Knowledge

Essential criteria
  • Demonstrate understanding of NHS CHC framework
  • Demonstrate research-based practice and up-to-date knowledge of nursing issues
  • Excellent communication and organisational skills
  • Excellent IT and documentation skills
Desirable criteria
  • Understanding of Mental Health Act 1983
  • Understanding of Mental Capacity Act 2005
  • Knowledge of Personal Health Budgets
  • Practical working experinence of setting up and managing Personal Health Budgets
