An opportunity has arisen for a Community Learning Disability Nurse to join a highly experienced, multi-professional health and social care care team working with people with learning disabilities in Stockport.
The post-holder will be based within the Community Learning Disability Team in Stockport.
Stockport Learning Disability team is a truly multidisciplinary team that works with individuals with a learning disability from the age of 18. We deliver high quality health care assessments and interventions to people with complex and challenging needs. The team comprises of community nurses, speech and language therapists, social workers, psychologists, assistant practitioners and psychiatry input. The team is located at Stopford House and works across the Stockport area.
Main duties of the job
Role Summary:
Delivery of evidence based health and behavioural assessments and interventions to adults with learning disabilities who have complex and challenging needs. You will be working in a multi-disciplinary context with the expectation to be person centred, creative and innovative in meeting the health needs of individuals whilst offering support to families and carers.
There will be the opportunity to work across a range of settings and with a range of provider services. There are regular opportunities for continuing professional development and for peer supervision with other Community Nurses working in the Directorate. We are eager to support continual professional development and access to additional training opportunities.
Key Responsibilities of the Post:
The post holder will work in partnership with all members of the Multidisciplinary Team to achieve the ‘Good Health’ objectives of ‘Valuing People’.
Support people with learning disabilities who may exhibit a range of challenging behaviours, which may include verbal and/or physical aggression.
To provide a safe, effective, responsive, efficient, timely, community nursing service to people with learning disabilities, their families and carers.
To undertake a range of assessments to individuals with a learning disability and complex needs. This assessment work requires a specialist knowledge base on which to practice.

To provide a safe, effective, responsive, efficient, timely, community nursing service to people with learning disabilities, their families and carers. To undertake a range of comprehensive assessments for individuals with a learning disability and complex and challenging needs. This assessment work requires a specialist knowledge base on which to practice.
To monitor and evaluate complex interventions and packages of care.
The post holder may be required to become a physical intervention trainer, this task
requires the trainer to have highly developed and co-ordinated physical skills.
The post holder will be expected to offer behaviour support training to identified others
including teams around the adult
The post holder may be required to provide advice to others regarding the management of complex behaviour
To develop a range of interventions and specialised care plans.
To be responsible for co-ordinating the provision of clinical care, within a defined
caseload accessing supervision from a designated nurse as appropriate.
To embrace the public health role, promoting health and wellbeing and where possible,
prevent ill health.
To support the work of the Community learning disability nurses, in reducing the health
inequalities faced by people with learning disabilities.
To work collaboratively with primary care and secondary care services to ensure the
health needs of people with learning disabilities are met.
The post holder will undertake an active role in identifying and managing risk.
The post holder may be required to participate in Safeguarding Adult investigations.
The post holder will be required to work in partnership with Social Services and Child Safeguarding.
To represent individuals as unique and valid citizens in the goal of inclusion.
The post holder will receive appropriate support to manage their own caseload and will be expected to prioritise competing demand
Person specification
Education / Qualifications
Essential criteria
- Registered Nurses Learning Disabilities (RNLD)
- Post registration training / willingness to undertake training plus experience to postgraduate diploma level.
Desirable criteria
- Clinical Supervision / Mentorship qualification.
Experience
Essential criteria
- Demonstrate knowledge and experience in supporting individuals who have a learning disability
- Experienced in delivering advice and support to a wide range of people.
- Experienced in partnership working.
- Experienced in managing risk
Desirable criteria
- Experienced in delivering training.
- Previous community nursing experience desirable
- Experience in supporting individuals with complex health needs
Knowledge
Essential criteria
- Established knowledge of the health inequalities agenda for people with learning disabilities
- Understanding of Person Centred Planning.
- Evidence of continued professional development.
- Knowledge of Child Safeguarding and Safeguarding Adult legislation
- Knowledge of current strategic drivers for community learning disability teams
Desirable criteria
- Knowledge of the evidence based around people who present with offending behaviour.
- Completed in depth training in Person Centred Planning.
