Job overview
An opportunity has arisen for a Health Care Support Woker to join the Physical Health and Treatment Team in Stockport.
Main duties of the job
The post holder will work as part of the Physical Health and Treatment Team to participate in the running and ongoing development of the Depot, Clozapine and Physical Health Check Clinics for Adult Mental Health Services in Stockport providing physical health care monitoring and support. There will also be a role in monitoring the efficacy of medication and reviewing side effects.
You will work across clinic and community settings as required delivering a high standard of care in accordance with the policies and procedures of Pennine Care NHS Foundation Trust

Detailed job description and main responsibilities
To support the monitoring of the effects of medication and side effects
To support and implement care that meets the mental, physical, spiritual, psychological and social needs of the patient, being sensitive to age, culture, race, gender, ethnicity, sexuality and disability.
To provide health promotion for service user’s in the Stockport Mental Health Service understanding the relationship between mental health and physical health.
To be able to enhance a patients overall wellbeing, and promote parity
of esteem.
To be able to use the POCHI machine and follow processes outlined in Clozapine Clinic procedures
To undertake venepuncture training and maintain knowledge and skills to be able to use this in both clinical and residential settings.
Person specification
education/qualifications
Essential criteria
- Good standard of general education including good numeracy and literacy skills.
- NVQ 3 or equivalent
- Commitment to undertake Continuing Personal Development
Desirable criteria
- Evidence of recent further development / training.
Experience
Essential criteria
- Experience of undertaking a supportive role.
- Experience of working with individuals with mental health needs
- Experience of working with individuals in distress
Desirable criteria
- Experience of lone working
- Working with groups/coordinating activities for service users
- Working with people with mental health problems
Knowledge
Essential criteria
- Understanding of mental health issues and how these impact on individuals
