Job overview

Caseload Coordinator - 30 hours per week

An exciting opportunity within the Stockport  Discharge to Assess Team and Intermediate Care at Home.

We need people who share our excitement, embrace different ways of working and are able to work flexibly across organisational boundaries .

The Discharge To Assess element is multi-professional services who assess patients upon discharge from hospital in the community .We provide individualised care to meet the person’s needs and facilitates transfer for onward care into the appropriate service: Health, Adult Social care or partner agencies.

The Rehabilitation Element This element incorporates a free, short-term rehabilitation service for adults to help prevent unnecessary hospital admission, enable early discharge from hospital and prevent unnecessary admission to long-term residential care. The service is for adults who have been assessed as medically stable and who do not require hospital care.

Main duties of the job

  • To undertake tasks which involve the management and maintenance of the teams clinical caseload for Senior Support Workers, including all associated administration tasks.
  • The job will include using a variety of computer based programmes including EMIS, Advantis and Microsoft Packages.
  • Liaise with different Professionals to coordinate the patients care.
  • Responsible for delegation and coordination of visits for support workers on a daily basis.

Detailed job description and main responsibilities

This comprises a multi-disciplinary integrated team consisting of highly skilled senior nurses, Occupational Therapists ,Physiotherapists, Social Workers and Senior Support Workers.

The whole service provides a person centred approach, which can be provided in a client’s own home or in a designated residential care home.

You will be working within a multi-professional team incorporating physiotherapists, occupational therapists, nurses, social workers, rehabilitation support worker and admin as well as liaising with other community health and social care teams.

The service is provided across 7 days a week covering core hours of 8am-9pm, this position involves working from 8am until 6pm. 

You will be based at Regent House, Heaton Lane, Stockport, SK4 1BS.


The successful candidate will be expected to work independently, but also as an integral member of the multidisciplinary team. You will be offered regular 1:1s. 

You will need to have an enthusiasm for working within the community. We place a great emphasis on training and personal development and will provide you with access to training and supervision. The candidate will be able to demonstrate  excellent time management and advanced communication skills.
There will be support available team members to support you to develop within your role .

To be an effective member of the Multidisciplinary Team.

Communicate effectively with staff patients and carers as required.

To complete all administration duties associated with the role 

To carry out other duties under indirect supervision of a registered professional.

If successful this vacancy will require an standard DBS check at a cost of £24.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Qualifications

Essential criteria
  • • Mathematics GSCE or equivalent or equivalent experience
  • • English GSCE or equivalent or equivalent experience
  • • Evidence of other educational achievements
Desirable criteria
  • NVQ level 3 or equivalent in Health and Social
  • Care Certificate
  • • Information technology Qualification.

Knowledge

Essential criteria
  • To have an understanding of Confidentiality
  • To have an understanding of current issues affecting the NHS and Social Care.
  • Knowledge of rota systems, word and excel spreadsheets
