Job overview

An exciting opportunity has arisen in Stockport to be part of a team of Kentown nurses working across Greater Manchester leading, developing and coordinating children’s community palliative and end of life care, promoting a holistic approach to 24/7 care and acting as a resource for multi-agency teams. 

Main duties of the job

The post holder will work as a Kentown nurse utilising knowledge and expertise in the care and management of children with palliative care needs. They will deliver safe, personal, effective and compassionate care to children, young people and their families, ensuring empathy and quality

As a senior nurse there is an expectation that the post holder will fully participate in clinical and professional leadership, the post holder should inspire, motivate and empower others; this will require them to be highly visible and to have an accessible approach for the children, young people and their families. The post holder will play a significant role in the education and training of staff, empowering them to care for children and young people and their families when life is limited or bereavement has occurred, liaising between community services, hospital and hospices.

Detailed job description and main responsibilities

·         Provide holistic and evidence-based nursing care that meets the child’s palliative care needs across the care pathway including those families facing the end of their child’s life; at the time of death and following death.

·         Assess, plan, develop, evaluate and record individualised plans of care to meet the needs of the child, young person and their families so that appropriate care and support is established and maintained as they move between hospital and community settings.

·         Collaborate with the team around the child to manage symptoms, taking urgent action for distressing and complex symptoms and situations

·         Recognise the impact of advanced disease or deterioration in child’s condition and apply judgement to evaluate the outcomes of care and interventions

·         Critically analyse complex clinical data and information to inform decision making

·         Provide clinical leadership in non-pharmacological symptom management

·         Work across organisational and professional boundaries, develop effective partnerships and joint working and contribute to achievement of consistency and equity in the clinical management of children and young people with complex and palliative care needs.

·         Work in partnership with other sectors and agencies including other members of the Kentown team to promote collaborative work across care settings to facilitate seamless care.

·         Provide support at home for children and their families, through the mobilisation and coordination of local services to support the patient and family during palliation and bereavement.

·         Implement the advance care plan, review and negotiate treatment goals, end of life decisions and ceilings of care (with support of an experienced specialist where necessary).

·         Follow and adhere to organisational guidelines/policies relating to supporting and caring for a child, their family and colleagues before, at the time of and after death.

·         Give advice to families and carers on practical issues following death of a child and signpost to appropriate help and support.

·         Respect the spiritual, cultural, religious and community needs of the child and family.

·         Have a commitment to collaborative working and support the transition process for young people with palliative care needs into adult services.

·         Practice in accordance with legal, ethical and professional principles, national and local policies and standards to provide safe, effective, timely, cost-effective palliative and end of life care to children families and carers

·         Contribute to data collection and audits required by Kentown support, organisation or nationally.

·         Evidence continuing professional development.

·         Recognise own limitations and seek appropriate emotional and developmental support when required.

·         Recognise own supervision needs and access bimonthly clinical supervision acting as a role model to support self and others within the MDT team. To participate in mandatory annual appraisal with line manager as per organisational policy.

·         Act as a facilitator and resource for colleagues who require bereavement support.

·         To attend core mandatory training and annual updates in line with Trust policy.

·         To take professional responsibility for prescribing, transcribing and administering of medication in accordance with [Trust] medicines management, controlled drugs and non-medical prescribing policies and the NMC Standards for Medicines Management.

·         To conduct oneself in accordance with the NMC Code of Professional Conduct and Trust policies.

·         To ensure your own NMC registration is maintained, and the mandatory three yearly revalidation is carried out in a timely manner.

 

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16
