Job overview

FULL TIME POST - 37.5 HOURS A WEEK

An exciting opportunity has arisen within the Colorectal Cancer Nurse Specialist team as a Cancer Care Coordinator. The role will involve supporting the Clinical Nurse Specialist team with the delivery of Personalised Care for Cancer.

Personalised Care for Cancer includes supporting the provision of Holistic Needs Assessments, the development of a supportive care plan and offering Health & Wellbeing advice and support.

In addition to this, the role will also involve supporting the implementation and administration of Personalised Stratified Follow-Up monitoring and tracking of surveillance tests and being a single point of access for patients. This will include rapid re-entry into the system for those people identified as having urgent or specialist needs.

Main duties of the job

You will work closely with all grades of medical and nursing staff and wider health care teams. The role requires a team player but also someone who can work independently and use their initiative – it is an excellent development opportunity for someone who is enthusiastic with excellent organisation, IT and interpersonal skills and who is passionate about ensuring the best possible compassionate care for people living with and affected by cancer.

Communicating effectively (daily and directly, face-to-face and telephone) with a range of individuals on various levels, e.g. patients, relatives, carers, other health/social care professionals and community workers is essential.

Detailed job description and main responsibilities

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require a Standard DBS check at a cost of £24.90. This cost will be deducted from your first Trust salary.

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Person specification

Education and Qualification

Essential criteria
  • GCSE English Language and Mathematics (Grade A-C) or equivalent
  • NVQ Level 3 in health related subject/equivalent knowledge and experience (eg Health and Social Care/Diagnostics and Therapeutics
  • Prepared to underatke relevant learning and development courses
Desirable criteria
  • NVQ Level 4 or additional training to Diploma level or equivalent
  • Nurse Associate

Knowledge

Essential criteria
  • Understanding of person centred care
  • Understanding of health and social care environment
Desirable criteria
  • Understanding of Macmillan Information and Support
  • Knowledge of relevant cancer/treatment.interventions and terminology

Experience

Essential criteria
  • Relevant health and social care experience at AfC Band 3 or equivalent
  • Experience of multi-professional working
  • Experience of data and database management
  • Demonstrate excellent communication skills
  • Experience of providing administrative support
Desirable criteria
  • Experience of coordinating a patient caseload
  • Experience of coaching /teaching patients and carers

Skills and Abilities
