Stockport Foundation Trust is one of four ‘specialist’ hospital sites in Greater Manchester. Being a ‘specialist’ hospital will enhance our general surgery, anaesthetics, critical care and emergency medicine for the benefit of people in Stockport, High Peak, Cheshire and across Greater Manchester.

Job overview

The successful applicant will be responsible as an advanced specialist for leading and  providing safe, efficient and cost-effective clinical pharmacy services to Transfer to Assess and Complex Discharge patients at Stockport NHS Foundation Trust.


Main duties of the job

  • ·      To ensure rationalisation of medication particularly for complex patients with polypharmacy, including rationalising medication timing to aid patient concordance for self-managed medication, and to help reduce the number of care calls required for patients with managed medication.
  • To liaise with the community health care teams about the patient’s            needs including ongoing monitoring requirements
  • To provide support and medication advice to other members of the            multi-disciplinary team
  • To use the skills as a pharmacist prescriber to write discharge                    prescriptions to meet service needs

Detailed job description and main responsibilities

  • To provide a highly specialised clinical pharmacy service.
  • Co-ordinate with the multidisciplinary teams to identify patients requiring pharmacy input
  • To ensure the pharmaceutical service facilitates early discharge where appropriate
  • To plan the day-to-day delivery of the pharmacy service to the Bluebell unit.
  • To work across traditional boundaries e.g. primary / secondary care to provide seamless care.
  • To use the skills as a pharmacist prescriber to write discharge                    prescriptions to meet service needs
  • To participate in the pharmacy departments on call and out of hours pharmacy services rota as and when required.

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Person specification

Education & Qualifications

Essential criteria
  • Vocational Master’s degree in Pharmacy (MPharm) or equivalent
  • Postgraduate UK Diploma or higher (MSc or Doctorate) in Clinical Pharmacy / Pharmacy Practice or equivalent or relevant experience
  • Non medical prescribing qualification
  • Annotation to the GPhC Pharmacist Register as an independent non-medical prescriber

Knowledge

Essential criteria
  • Advanced level of clinical knowledge
  • Evidence of use of clinical IT systems

Experience

Essential criteria
  • • Experience in Clinical Pharmacy services in a UK hospital
Desirable criteria
  • • Experience in delivering training.
  • Previous experience of working in a clinic setting

Skills & Abilities

Essential criteria
  • • Demonstrate a good level of verbal and written English.
  • • Highly developed communication skills
