Job overview

Occupational Therapist- Discharge to Assess Services and Intermediate Care at Home 

The Discharge to Assess team is an innovative and exciting service, aiming to facilitate timely discharges to the residents of Stockport and surrounding areas. Working as part of a multi-disciplinary team, they will facilitate timely discharges from acute hospital sites and provide assessment and support planning to meet a person’s needs in the community. The post holder will provide and/or arrange appropriate support interventions in the community via Discharge to Assess collaboratively working closely with colleagues across all sectors.

Working as part of a lively and dynamic multi-professional team, you will deliver holistic support and interventions to improve the health and well-being of individuals in a bed based facility. In joining our team you will be helping us to drive our ambition of improving the health and well-being of people by working across organisational boundaries.

This is an exciting time to join our lively and diverse team, as the service expands and develops we require enthusiastic staff to shape our evolving service using skill, motivation and person centred care to provide an innovative service to the communities of Stockport.

Main duties of the job

We are looking for highly motivated, forward thinking and experienced clinicians with a specialist interest in frailty who will work alongside an integrated Multidisciplinary Team to improve flow and quality of the patient's journey. You must have excellent communication, organisational and leadership skills, with the ability to work autonomously, whilst being flexible and adaptable to supporting the service leads in developing and changing the service as it evolves.

You will be part of an integrated therapy team and we have a strong commitment to support all staff members with CPD opportunities. We offer regular supervision and annual appraisal with a regular in-service training programme.

The post requires a commitment to weekend working which is underpinned by teaching and a competency programme.

The post requires working hours over a 7 day service and fulfilling the service needs until 9pm.

Detailed job description and main responsibilities

Qualified Occupational Therapist

The Discharge to Assess team continually develop and enhance the seamless transfer of care from acute care settings to a patient's usual residence. The successful candidate will therefore embody trust values, in supporting and facilitating safe discharge to individuals who are deemed medically optimised. The successful candidate will be passionate in providing community based interventions, demonstrating a high level of holistic care independently and as part of a team.

The Multi professional team consists of highly skilled, registered Nursing staff, Occupational Therapists and Physiotherapists, Pharmacy and Community Support Workers.

The team provides a seven day service until 9pm, therefore this entails working some weekend and bank holidays. The post is based in the community delivering assessments and interventions where required; therefore independent travel with access to a vehicle, business insurance and a full driving licence to deliver the role is essential. 

We promote a positive ethos on personal and professional development, providing access to training and supervisions. You will be offered induction, supernumerary working and full access to mandatory training, furthermore CDP training is full supported and very much encouraged. Regular PDRs and reflective supervisions are provided, alongside peer support sessions in the form of multidisciplinary and profession specific team meetings.

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Education & Qualifications

Essential criteria
  • • Diploma/Degree in Occupational Therapy.
  • • State Registration with the Health & Care Professions Council.
  • • Commitment to lifelong learning – evidence of CPD maintained in a portfolio.
Desirable criteria
  • • Experience as clinical educator.

Knowledge

Essential criteria
  • • Specialist knowledge and application of occupational therapy assessment and interventions.
  • • Ability to use and interpret a range of standardised assessment.
  • • Use of OT outcome measures
  • • Basic I.T. skills.
Desirable criteria
  • • Appraisal Skills training.
  • • Clinical Supervisor training.
  • • Introducing management training.
  • • Clear vision of role/commitment to specialty.
  • • Evidence of some area of special interest.

Experience

Essential criteria
  • • Post-registration experience of working with a variety of conditions.
  • • Supervision of junior staff, assistants and students.
Desirable criteria
  • • Involvement in relevant audit.
  • • Leadership and first hand management experience.
