This is an exciting opportunity for a respiratory physiotherapist to lead on the delivery of a community Pulmonary and Heart Failure Rehabilitation service with Stockport COPD and Oxygen services.

Working with an established respiratory health care team, you will bring your expert skills in respiratory disease and rehabilitation management to the service and provide excellent care to the patients in Stockport.

To enhance the Stockport Pulmonary and Heart Failure Rehabilitation Team we are seeking to recruit a physiotherapist with the relevant experience to undertake the role of leading and developing our community respiratory and heart failure rehabilitation programme. We are a dynamic, forward thinking team, providing care for . The post will be an addition to the current team which consists of COPD Specialist Nurses, Respiratory Physiotherapists with an interest in oxygen and Heart Failure, a GP with Special Interest in Respiratory Medicine, Clinical Support Workers and Administration Support.

This will also require consideration of advanced therapies, delivering patient education and providing support, aiming to reduce symptoms, prevent unnecessary hospital admission/ re-admission and maximise patient’s quality of life.

The service delivers the Pulmonary and Heart Failure rehabilitation across  two community venues and occasionally patients' homes. This requires flexibility within the role and a wide range of skills and experience in caring for pulmonary disease and heart failure patients.

There is also a teaching and resource requirement to share your expert knowledge with the patients attending the programme and networking with other rehabilitation programme providers in the North West.

Significant clinical experience at in the field of pulmonary and heart failure rehabilitation is essential, and will be assessed at interview using clinical scenarios. Therefore possession of knowledge and skills caring for patients with chronic lung disease and heart failure is essential. We would like to see evidence of innovation and a commitment to clinical improvement projects which aim to both enhance patient’s experience and promote positive patient outcomes.

You will have excellent communication and interpersonal skills, clear decision making capabilities, and be a vocal advocate for patients. You can expect to be part of a constantly evolving proactive, dynamic and committed multidisciplinary team, offering on-going support and an initial induction period.
