Job Summary:  

 To Train as a Specialist Community Public Health Nurse –School Nursing

To work alongside the Practice supervisor and Practice Assessor to develop enhanced skills in Specialist Public Health Community Nursing.

 Complete SCPHN Apprentice course at level 7. Meeting the required Apprenticeship Standards, University Academic Standards and the NMC Standards for qualification.

 The apprenticeship programme combines and integrates both academic and work-based learning through close collaboration between employers and education providers.

 50% will be on placement and you will be allocated a Practice Assessor and Practice Supervisor to guide and assess your development. 50% will be University time which will include study days and regular academic assessments.

 

Main duties of the job

To work alongside the Practice supervisor and Practice Assessor to develop enhanced skills in Specialist Public Health Community Nursing. 

 Complete SCPHN Apprentice course at level 7. Meeting the required Apprenticeship Standards, University Academic Standards and the NMC Standards for qualification.

 The apprenticeship programme combines and integrates both academic and work-based learning through close collaboration between employers and education providers.

 Whilst an apprentice you will retain your requirement to adhere to the NMC standards for registration and practice as a registered Nurse, ensuring the safety of clients and staff and acting as a role model to others.

 Be a reflective practitioner working within your own capabilities utilising evidence-based practice.

Detailed job description and main responsibilities

Work with children, young people and families and establish relationships to explore solutions, plan interventions, safeguard and promote Public Health based on assessment alongside colleagues within Stockport Family.

 Communicate any concerns or difficulties via the appropriate route.

Work and develop alongside the integrated Stockport Family Team.

 Submit confidential information and reports, as requested, under the supervision of the assessor or supervisor.

Deliver evidence-based practice appropriate for the service.

 Support the delivery of the Healthy Child Programme and assess child development, following pathways of care when need is identified.

 To provide SEND advice, guidance and support when identified need emerges.

 Be part of a locality multi-disciplinary team providing integrated early help and support with a focus on Public Health.

 To undertake assessments and interventions related to mental health and emotional wellbeing, parenting concerns, family relationship breakdown, safeguarding, child health and development, infant feeding, childhood nutrition and medical needs in community settings.

 Prepare and record assessments, case notes, contacts, and reports accurately and promptly in accordance with record keeping and trust policies and national protocol and guidance as relevant.

 To advise the caseload holder and Vulnerable Children’s Team (VCT) of suspected abuse and safeguarding issues adhering to Stockport Multiagency and Stockport Foundation Trust Policies and procedures to ensure safety and welfare of children and families.

 Participate in group and individual supervision, including safeguarding supervision in relation to casework and specific practice interventions and group programmes.

As your skills develop you will be expected to manage with supervision a workload within individual and team priorities adhering to trust policies.

 Work collaboratively under supervision with partners and contribute to multiagency meetings and planning.

 As your skills develop you will be expected to undertake with supervision the role of lead professional, undertake early help assessment and planning, chair team around the child/family meetings where appropriate and support universal services in the completion of high-quality assessments.

Work in a person-centred way and always consider the voice of the child, young person in the context of the family and community functioning.

 Work in a variety of settings including family homes, schools, community settings and clinics.

 Develop knowledge of the local community areas, including local community assets, voluntary and faith groups and support equality, diversity and SEND needs within community settings.

 Undertake relevant administrative duties and maintain appropriate records in accordance with the role and statutory requirements.

 To adhere to the Trust, NMC, Apprenticeship and University standards for training.

 Attend all sessions and document placement and public health days appropriately on the electronic portfolio.

 Ensure the Practice Supervisor and Practice Assessor are aware of your whereabouts for safety and monitoring.

 To ensure all mandatory training and role specific training is up to date.

 To ensure Level 3 safeguarding training requirements are kept up to date.

If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Qualifications and Education

Essential criteria
  • • Current First Level Registration on Parts 1 or 2 of NMC register or be expecting to complete by August 2025
  • • Up to date professional portfolio demonstrating evidence of reflective, evidence-based practice.
Desirable criteria
  • • Level 6 or 7 study
  • 120 academic credits at Level 5 or APEL equivalent

Knowledge

Essential criteria
  • • Demonstrate knowledge of nursing and evidence-based practice
  • Demonstrate knowledge of the Healthy Child Programme
  • • Demonstrate knowledge of child development, SEND, health, and well-being in the age range for pathway chosen.
  • • Demonstrate awareness of the potential future role for the pathway chosen
  • • Demonstrate Political Awareness
  • • Demonstrate understanding of the Principles and Values underpinning the NHS Constitution
  • Demonstrate an understanding of the Trusts values
  • Demonstrate IT capability
