We have an exciting opportunity for a Nurse Practitioner to join us in the Urgent Community Response/Virtual Ward Team.
We are looking for a dynamic, enthusiastic practitioner to support the delivery of the 2 hour Urgent Community Response and Virtual Ward mandates to improve out of hospital care and support the management of patients in their own home.
The team provides urgent care to people in their homes which helps to avoid hospital admissions and enable people to live independently for longer. The team enables older people and adults with complex health needs who urgently need care, to get fast access to a range of health and social care professionals within two hours.
It is an exciting time to join us as we expand the Virtual Ward/Hospital at Home service.
Virtual wards provide hospital level care and remote monitoring in patients homes allowing them to stay at home where they are most comfortable.
There is emerging evidence to show that patients have better outcomes when treated at home, rather than being treated in hospital.
The post will work alongside a multidisciplinary team consisting of GP's, ACP's Senior Nurses, Therapists and Health Care Assistants utilising digital technology.

Monitor and improve standards of care through supervision of practice, clinical audit, evidence based practice, learning and development and the provision of skilled clinical leadership

• Assess patients within a Crisis situation and offer appropriate nursing assessment intervention, and review to meet their needs using advanced clinical assessment skills
• Assesses, plan, implement and reviews routine through to complex nursing care plans using research and evidence based knowledge to underpin practice and which support avoiding a hospital admission.
• To supervise, lead, inform and delegate staff as appropriate to the needs of the service by utilising skills and resources effectively as directed by the Urgent Community Response team leader.
• To be an expert resource within the team regarding all aspects of nursing care.
• Demonstrates a high level of intuitive and creative problem solving skills to facilitate patient centred care; rationalising information to create patient concordance by using a high level of communication/negotiation skills to affect the outcome.
• Participates in the Performance and Development Review process within the team and undertakes review of junior staff under supervision of the manager
• To participate in the investigation of complaints and safeguarding concerning nursing services in accordance with the level of experience and encourage comments and suggestions on improving the quality of the service.

If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
