Job overview

An exciting opportunity  has arisen to work as part of the Digital Hub team. The successful candidate will participate within duties aligned to triage/navigation and assessment and digital remote monitoring  conjunction with an administration team, and will liaise key stakeholders across primary, community and secondary care. They will work in collaboration with the Out of Hospital teams..
Excellent communication and IT skills are required to support the Digital Hub functions of navigation and virtual remote monitoring. This will include prioritising patients to the right team and services and to be competent in the utilisation of digital remote monitoring and virtual patient consultations. They provide excellent professional, skilled and effective person centred, evidence-based nursing care. They are actively involved in promoting self-care and promoting independence.
They contribute to achieving all performance indicators, essential standards and quality outcomes They contribute to the education and development of the workforce and students. They contribute to staff appraisals and support personal development plans.
The Registered Nurse has sound knowledge of trust policies and practice guidelines and demonstrate compassionate care that is based on empathy, respect and dignity.

This role is within a digital hub where the candidate will be navigating health care professional referrals to the appropriate  service and monitoring patients via the virtual ward.  

Main duties of the job

Undertake holistic patient assessment that will support care navigation as appropriate
Implement and evaluate individual treatment plans for patients within the digital platform and virtual ward.
Produce and maintain accurate records both written and electronic in a timely manner. To abide by all relevant policies including Information governance, confidentiality and data protection.
Understand own responsibilities and accountability in the delivery of care to patients, working within own competence and professional code of conduct.
To communicate with patients, carers and staff as part of effective patient care and in line with digital; remote monitoring guidelines
To act as Assessors or Supervisors for Pre-Registration Nursing Students, Trainee Nurse Associates / Trainee Assistant Practitioners and other learners in practice as appropriate or junior members within the digital team.

Detailed job description and main responsibilities

To communicate with patients, carers and staff as part of effective patient care and in line with digital monitoring guidelines and clinical templates. To lead the team in the absence of the SPOA Team Leader.


Produce and maintain accurate records both written and electronic in a timely manner. To ensure that the digital platform has accurate clinical recording alongside EMIS Community. To abide by all relevant policies including Information governance, confidentiality and data protection.

 

To engage in partnership working with patients/carers and families to facilitate person centred holistic care.
Understand own responsibilities and accountability in the delivery of care to patients, working within own competence and the NMC Code (2015)
To have an understanding of the health conditions of patients and to recognise common signs of deterioration or exacerbation of those conditions. To ensure accurate electronic records are completed within the Digital templates upon the platform and EMIS Community.

Person specification

Education & Qualification

Essential criteria
  • Current 1st level registration with NMC Diploma/ degree in Nursing Practice
  • GCSEs Grade C or above in Maths and English Language / New GCSE Grade 4-9 in Maths and English Language OR Functional Skills Level 2 in Maths and English or Equivalent (not entry level 2)
Desirable criteria
  • Teaching and Assessing Certificate

Knowledge

Essential criteria
  • Demonstrate knowledge of current local and national health agenda and developments.
  • Awareness of current clinical issues in out of hospital care and understand the relevance of virtual wards and remote monitoring
Desirable criteria
  • Demonstrate an understanding of an implication of an issue which could impact on community service provision.

Experience

Essential criteria
  • Delivery of patient
Desirable criteria
  • Evidence of teaching and assessing students and junior members of staff
  • Developing areas of expertise in community related practice. For example community nursing/ Frailty/ End of Life care
  • Experience of audit involvement in project work.
