Job overview
We are looking for a care coordinator to join our friendly and welcoming Early Intervention for Psychosis team in Salford. You will be joining a large multi-disciplinary team with a wide range of knowledge and experience who will support you in your new role.
Main duties of the job
As a care coordinator, you will work under the CPA framework as this tranforms into the Community Mental Health Framework. You will manage a caseload of people who have been assessed as experiencing their first episode of psychosis and supporting them as they navigate this journey.
You will contribute to the assessment and duty rotas, and you will receive regular supervision and peer support.

Person specification
Qualifications
Essential criteria
- Nursing (RMN, CPN), OR Social Work OR Occupational Therapy
Desirable criteria
- Evidence of robust continued professional development including training in relevant subjects
Experience
Essential criteria
- Demonstrable experience of working in the NHS either within a volunteer or paid post.
- Experience of working with people with complex presentations and high rates of co-morbidity
- Experience of work within a multi-disciplinary team
Desirable criteria
- Demonstrable experience of working with people with psychosis & mental health difficulties such as depression and anxiety
- Experience of working within a CMHT / EIS setting; managing a caseload of service users and completing assessments with people who have been referred into the service
