An opportunity has arisen for an experienced community mental health nurse to join our dynamic, supportive CMHT at Prescott House in Little Hulton, Salford as Senior Nurse Practitioner.

Having community experience of nursing people with complex mental health and social needs, you will be running a clinic to manage care, treatment and risk of an established  standard care caseload.

If you have NMP already, this would be great but GMMH will support you to achieve this if not.

This senior role demands leadership and support for a team of staff, involving line management supervision, supporting cpns and clinics, duty support, recruitment, supporting trust medication and physical health agenda, and deputising in the absence of the team manager, with senior roles for daily meetings and a bronze on call and on site role on a rota basis. evenings, weekends, bank holidays.

The ideal candidate will be working at or towards leadership level and have a passion for providing high quality care to service users, carers and families, supporting peers and inspiring this in others. 

Main duties of the job

Senior nurse in the team - supporting staff and peers to deliver high level, responsive care to service users, families and carers. Liaison with referrers, other services to meet service user needs and manage risks.

You will manage an established  caseload of complex service users under standard care and treatment, providing them with high quality interventions from within or without of CMHT to facilitate stability, safety and recovery, in conjunction with the service user/carer/family/mdt/other services as appropriate.

You will line manage junior nursing staff, and contribute to the safe and effective day-to-day running of the service and support CPNs and clinics within the CMHT.

Detailed job description and main responsibilities

1.    Provide nursing and clinical leadership and supervision, support and consultation to MDT staff in the team, in line with clinical pathways and the standard operating procedure.
2.    Undertake liaison and develop effective working relationships with GMMH and statutory and non-statutory partners.
3.    To maintain own clinical and professional competence.
4.    To participate in the service Bronze on Call/Bronze on Site rota.
5.    Deputise for the Team Manager in his/her absence and take on any delegated duties as required.
6.    Manage and evaluate a caseload in line with relevant clinical pathway as outlined within the operational procedure and will include: -
• Assessment
• Formulation
• Therapeutic intervention as required
• Signposting
• Close working with all colleagues
• Being responsible for a complex caseload
7.    To ensure services users/carers under the team  receive a high standard of physical healthcare.

Qualifications

Essential criteria
  • Registered nurse holding valid registration with NMC
Desirable criteria
  • further qualifications or evidence of post-registration study in a subject relevant to the specialty of health and social care, leadership or specialist practice.
  • Nurse Medical Prescribing Qualification

Experience

  • Experience of working in a community mental health setting
  • Detailed knowledge of best practice, legislation, governance and risk management relevant to post
