Job overview
The newly created Digital Health Service will be responsible for the clinical assessment and monitoring of Virtual Ward patients alongside the senior clinical assessment and care navigation of the acutely unwell
adult that has been referred via the Urgent Single Point of Access
The post holder will be responsible for managing the team which support remote monitoring of patients upon the Virtual Ward and offer senior clinical care navigation.
The post holder will be responsible for ensuring that all digital platforms are operational and be compliant
to the service standard operating procedure.
The post holder will be responsible for ensuring the timely assessment and appropriate care navigation of
acutely ill patients that are referred via the SPOA Is completed using the appropriate clinical assessment
tool for triage and carer’s needs.
Main duties of the job
In conjunction with the Pathway Lead – Out of Hospital, Urgent Community Services, provide effective
leadership, monitoring of professional and care standards, and effective use of resources within the
Digital Hub
To operationally manage the Digital Hub team and deputize for Pathway lead when required.
Support and develop health and social care integration across sharing responsibilities for leadership and
management of staff in multi-professional teams.
To be a visible and accessible presence in the clinical area where appropriate
To be actively involved within the clinical care navigation and digital monitoring of patients within the
caseload.
To act as role model and mentor
To understand the clinical pathways aligned to Virtual ward and ensure the deteriorating patient is
appropriately escalated
To ensure clinical competency frameworks are in operation
To be responsible for ensuring the provision of compassionate, safe and high quality patient care
Actively contribute to and support agreed developments within the Directorate with focus for supporting
clinical effectiveness and innovative practice

The post holder will be responsible for ensuring the timely assessment and appropriate care navigation of
acutely ill patients that are referred via the SPOA Is completed using the appropriate clinical assessment
tool for triage and carer’s needs.
The post holder will be responsible for offering clinical leadership within the boundaries of the specific
profession and offer managerial and operational support to the registered and non-registered members of
the team.
The Digital Hub Team Lead has a fundamental role in ensuring the caring environment is conducive to
wellbeing and the effective delivery of budgetary, workforce and resource management.
The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.
If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
