Job overview
This is a 6mth Contract - secondment of fixed term contract to support the Pathway leads within the Directorate of Out of Hospital
The Directorate of Out of Hospital, Urgent community Services is looking for a motivated, experienced clinical to join their senor leadership team. The post will be for 6months on a secondment basis or fixed term contract. This post will be pivotal in supporting the current leadership during periods of significant transformation work.

Main duties of the job
Pathway lead will have identified services within the Directorate and will:
Ensure the efficient and effective running of the services.
Supervise the provision and delivery of the pathway services by working in collaboration with Clinical Leads, Team Lead and team members.
Develop and implement operational management systems and process pathways specific for the individual services patients, in collaboration with services within and external to the Organisation.
Work alongside the Team Lead and/or Clinical Leads offering critical challenge within current practice, thus ensuring that all care delivered is evidence based, effective and efficient.
Ensure that all care provided by the team members is evaluated and reviewed and efficiency maximised.
Adopt a LEAN methodology and philosophy.
Lead with key stakeholders on the identification, development and evaluation of operational objectives and create the appropriate SOP’s, protocols, and strategies to maintain efficiency and effectiveness

Detailed job description and main responsibilities
Pathway lead will have identified services within the Directorate and will:

To support the Directorate Manager in developing the operational direction of the identified services.
Actively support the Organisations Values and Beliefs and share responsibility for the provision of the pathway model services.
To operationally manage identified service/services within the directorate, working alongside other key members to ensure that all members are governed within their professional boundaries.
Work collaboratively with colleagues and managers to develop partnerships within the integration programme for Stockport, ensuring a co-ordinated approach in the delivery of the Services.
To work collaboratively with multi agencies and ensure that all pathways adopt the health/social care cohesive partnership wherever possible, and work in collaboration with the Clinical Leads in relation to the quality agenda.

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Education & Qualifications
Essential criteria
- Registered Professional RN/AHP
- Project management qualification or experience
- Postgraduate study at advanced level, or equivalent experience
- Time management and organisational
Knowledge
Essential criteria
- Understanding of clinical risk and incident management, patient safety and clinical quality improvement initiatives
- Evidence of audit, and action planning
- Ability to demonstrate solid understanding of Primary Care and secondary care interface
- Evidence of continuous professional development and practical application to work place
