Job overview
An exciting opportunity has arisen for a Band 6 nurse to work as an integral part of the Knowsley Community CVD Service. This is a 12 month opportunity in which time you will be a key member of the team. Responsible for the development, coordination and delivery of a comprehensive programme of Cardiovascular Rehabilitation. The programme will be delivered to the Residents of the Borough of Knowsley in a variety of Community settings, with specific emphasis on the delivery of a comprehensive menu based programme of cardiovascular rehabilitation. The post holder will contribute to the management, planning and education of cardiovascular patient’s complex health and well-being needs in line with the Patients Vision.
Main duties of the job
You will maintain and utilise advanced clinical skills to analyse and interpret history of illness, presenting symptoms and physical findings to enable diagnosis, planning and treatment of the patient.
Plan towards the delivery of high quality CVD rehabilitation programme
working with the Community CVD MDT and multi-disciplinary agencies.
According to patient’s needs, review patients on home visits or in clinic to
provide appropriate levels of follow up care post discharge. To assess individuals physical, social, psychological and emotional needs prior
to commencing the rehabilitation programme.
To offer ongoing support in health promotion and healthy lifestyle choices. To have a good understanding of monitoring and prescribing a menu based approach, supporting the patient to reach their goals.
To ensure that all patients progress through the relevant stages of the
programme at the correct time and that appropriate information and
advice is given at each stage.
To ensure that all patients are offered appropriate care via an exit strategy for promotion of continued lifestyle change within their local
community.

Detailed job description and main responsibilities
- JOB SUMMARY
To work as an integral part of the Knowsley Community CVD Service in the development, coordination and delivery of a comprehensive programme of Cardiovascular Rehabilitation. The programme will be delivered to the Residents of the Borough of Knowsley in a variety of Community settings, with specific emphasis on the delivery of a comprehensive menu based programme of cardiovascular
rehabilitation. The post holder will contribute to the management, planning and education of cardiovascular patient’s complex health and well-being needs in line with the Patients Vision.
To contribute to the management, planning and education for patients with cardiovascular disease, ie stroke, heart failure, respiratory and health and well-being needs.
2. CLINICAL RESPONSIBILITIES - Maintain and utilise advanced clinical skills to analyse and interpret history of illness, presenting symptoms and physical findings to enable diagnosis, planning and treatment of the patient. Contribute to the management, planning and evaluation of the CVD patients with complex clinical and social needs.
- Registered practitioners who are non-medical prescribers are accountable for their prescribing activities by ensuring they are aware of their Professional accountability and any restrictions or special considerations in their prescribing practice. To note they act as a Prescriber within the whole clinical team involved. To be aware of, and adhere to, the NMC Standards for the Administration of Medicines Act 1992, and the Misuse of Drugs Act 1971.
- Plan towards the delivery of high quality CVD rehabilitation programme working with the Community CVD MDT and multi-disciplinary agencies.
- According to patient’s needs, review patients on home visits or in clinic to provide appropriate levels of follow up care post discharge.
- Address psychosocial-economic needs of both patients and carers and refer onto other agencies as appropriate.
Cardiovascular Rehabilitation Programme: - To undertake patients reviews in the home or clinic area and offer ongoing support in health promotion and healthy lifestyle choices.
- To be involved, and take an active role in all aspects of service
delivery – including the operational aspects of organising the
programme. - Clinical evaluation, exercise, and education/relaxation sessions.
- Have a good understanding of monitoring and prescribing a menu
based approach based upon sound evidence and best practice
guidelines and supporting the patient to reach their goals. - Ensure that all patients progress through the relevant stages of the
programme at the correct time and that appropriate information and
advice is given at each stage. - Ensure that all patients accessing the menu based approach for
cardiac rehabilitation are offered appropriate care via an exit strategy
for promotion of continued lifestyle change within their local
community. - To be responsible for contacting patients to arrange bookings,
correspondence and dealing with cancellations/rebooking
h. Assess individuals physical, psychological and emotional needs prior to commencing the rehabilitation programme.
3. MANAGEMENT RESPONSIBILITY
To demonstrate leadership by means of: - Effecting change within own sphere of practice when and where
indicated. - Making recommendations to improve services Influencing, motivating, supporting and enabling others to contribute towards the effectiveness and success of NHS Knowsley.
- Ensuring that all patients progress through the relevant stages of the
programme at the correct time, and that appropriate information and advice is provided at each stage. - To actively participate in Local and National Cardiovascular networks.
- To act as patient advocate and ensuring effective partnership work within the multi-disciplinary team.
- To contribute to the commissioned service to deliver to high quality Service .
- To ensure Key performance indicators of the service are met via service delivery and data recording.
4. EDUCATION AND TRAINING - To assist in the development of care pathways .
- Contributing to the implementation and supervision of planned student placements.
- Assisting in the delivery of in-service training programmes for CVD rehabilitation.
- Maintaining sound knowledge and understanding of issues relating to the prevention, treatment and rehabilitation of CVD.
- Demonstrate critical thinking and reflective skills to ensure own professional development.
- Participate fully in the clinical supervision sessions arranged by the team leader and clinical supervisor.
5. PROFESSIONAL RESPONSIBILITIES - Qualified Nurses are required to evidence maintenance of their registration with the Nursing and Midwifery Council (NMC) and work to the NMC Code of Professional Conduct.
- To participate in research and clinical audit for the cardiac rehabilitation service.
- To liaise with all members of the multi-disciplinary team relating to patient care and treatment plans.
- Responding to requests for advice and information from the public and other health professionals.
