The role of the End of Life & Bereavement Specialist Nurse is to take a dedicated role in the implementation of the SWAN model of care, the SWAN end of life and bereavement model is patient and family focused and centers on meeting the unique needs of each individual and their loved ones at the end of life and after death. It aims to provide excellent, individualised care to every patient and their families. Provision of immediate and on-going emotional support to the recently bereaved throughout the Trust and Merseyside area.
The responsibilities include advising, educating and implementing the model throughout the Trust. This will provide assurance that statutory duties for SWAN, End of life and Bereavement as stipulated in primary legislation, government strategy and national, regional and local guidance.
The role also involves staff supervision; practice development and case work management across the Trust and ensuring clinical staff are competent in this specific area of practice.

Meeting the unique needs of each individual and their loved ones at the end of life and after death,
Work autonomously, making critical decisions and judgements related to the management of End of life, Bereavement Care and the SWAN model, including but not exclusive the writing of reports to support the decisions made.
Communicate sensitive and complex information in health and multi-agency forums
To promote a proactive approach to SWAN, End of life, Bereavement Care across all services
Work in partnership with all the multi agencies and organisations involved in End of Life and Bereavement Care, by attending and participating in relevant meetings or sub groups to ensure strategic planning and development of services.
Policy Development, Implementation and Evaluation of the Bereavement role and the SWAN model of care
Contribute to the annual report for SWAN, End of life and Bereavement model to Quality committee and Trust Board.
To continually provide the delivery of training around SWAN, End of Life & Bereavement Care as appropriate in line with National and Trust guidance
Actively partake in clinical audit and research pertaining to end of life and bereavement care

Support the Development, implementation and evaluation of a comprehensive range of policies, standards and procedures which ensure the Trust is able to discharge their responsibilities relating to the requirements of national, regional and local guidance and legislation.
Ensure that the Trust maintains a robust Bereavement and SWAN referral process that is fully compliant with national and local guidance.
Work in partnership with all the multi agencies and organisations involved in End of Life and Bereavement Care, by attending and participating in relevant meetings or sub groups to ensure strategic planning and development of services.
To promote a proactive approach to SWAN, End of life, Bereavement Care across all services
Work autonomously, making critical decisions and judgements related to the management of End of life, Bereavement Care and the SWAN model, including but not exclusive the writing of reports to support the decisions made.
Utilise the Liverpool Quality Assessment (LQA) to maintain a robust monitoring process for SWAN, End of life and Bereavement Care, which promotes improvement and informs the Trust of areas of good practice.
Support monitoring the quality and efficiency of SWAN, End of life and Bereavement Care arrangements within the Trust, establishing the appropriate key performance indicators to demonstrate this.
Work closely with all members of the multi-disciplinary team to identify areas for improvement for SWAN, End of life and Bereavement Care, utilising the LQA action plan to address the issues and drive change
Support Trust compliance with the Care Quality Commission quality and performance standard requirement in relation to SWAN, End of life and Bereavement Care.
