This is a replacement post to provide a Consultant Respiratory Physician with special interest in Sleep and Ventilation. to complement the existing teams. The post will be based at the Aintree hospital site.

This is a full-time post attracting an initial 10 PA’s, including on call work (weekends and overnight non-resident on calls). Applicants interested in working less than 10 PA’s will be considered. The job plan for this role is an example of a 10 PA role; the job plan will be agreed with the successful candidate when appointed. 

Applicants are required to be fully registered with the General Medical Council, with licence to practise, and are required to hold membership of the Royal College of Physicians UK (or equivalent).  They should be listed on the Specialist Register for Respiratory Medicine or be due to be registered within six months of the date of appointment.

It is expected that the post holder will be willing to be flexible in terms of their working hours within the agreed contract.  This will include working unsociable hours, weekends and bank holidays.  Even though unsociable hours may be rostered in the job plan, the work will not include duties that are inappropriate for a consultant; furthermore, the rota will not contravene the hour limits of the European Working Time Directive.  On call will be non-resident.

The Post 
This is a role for a candidate with special interest in Sleep and Ventilation Respiratory Medicine with a wide range of inpatient and outpatient activities. Core duties include: 
•    Responsibility for inpatients on the Ventilation Inpatient Centre including 4 regional weaning beds 
•    Inpatient ward rounds at Southport Spinal unit giving advice and support for long term weaning 
•    Being part of the regional Sleep service and able to give advice 
•    Delivering specialist sleep clinics 
•    Provision of inpatient respiratory advice and referrals (for respiratory medicine) 
•    On call for Respiratory medicine including weekend on call 
 
 The post holder will join the consultant body within Aintree Chest Centre.

Detailed job description and main responsibilities

10 PA Job Plan  

  • 10 PA job plan, including 0.7 PA for respiratory oncall duties, time off is given in lieu for any work carried outside of these times including a bank holiday 
  • 1.5 SPA provided on appointment
  • Annualised job planning with: 
  • Inpatient ward weeks
  • Posttake ward round weeks (to maintain general respiratory knowledge and allow insight into the general functioning of the trust as a whole)
  • Outpatient clinic weeks Indicative schedules are negotiable and dependent on service needs. 

 OnCall Duties 

  • Approximately 1 in 16 weekdays non-resident on-call 
  • 1 in 8 weekends (2 consultants)
  • Nonresident with attendance for weekend and Bank Holiday posttake ward rounds
  • Average attendance time: 4-6 hours/day
  • Supported by junior medical staff and ANPs
  • currently attracts 0.7 PA and will be within the 3% banding for on-call payments 

 Sleep Service 

Starting in 1984 Aintree has pioneered and developed services in Sleep Medicine particularly sleep disordered breathing and has a specially commissioned comprehensive sleep diagnostic and management service. According to GIRFT we are the second largest Sleep service outside of London with a service for non-respiratory sleep disorders in place since 2004. We have a national reputation for sleep service development, innovation and education particularly utilising innovative software The service has 7 physicians and a comprehensive modern fully equipped sleep lab offering continuously oximetry, single band polygraphy, complex respiratory polygraphy and full polysomnography. It also offers MSLTs and is a treatment centre for narcolepsy. The department is active in research and encourages this. It contributes to investigator lead studies and therapy studies from industry pharmaceutical and CPAP and ventilator manufacturers.

Facilities 

Our state-of-the-art eight bed sleep unit opened in 2012. 

We are able to provide individual en-suite in-patient facilities for complex sleep study investigations, including four polysomnography beds to allow sleep staging by electroencephalogram and video recording. 

Sleep studies for sleep breathing disorders are usually carried out at home using simpler portable equipment to measure breathing patterns and oxygen levels during sleep. 

Ventilation Service 

The ventilation service also offers comprehensive ventilation services. It started offering acute NIV in 1996 and at that time 14 people were on long-term NIV. The department has grown and now offers comprehensive ventilation services which are centrally commissioned. We have 1200 people on long-term ventilation with about 30 of these on invasive ventilation. Inpatient care is focussed on the Ventilation Inpatient Centre this has 12 beds the unit is currently experiencing a ground up complete rebuild and we expect to move back there in May 2026 this is situated on the first floor above the main entrance and next to critical care. 

Liverpool Ventilation Service seeks to provide exceptional care for people requiring acute and long-term ventilation. We have a team of highly skilled clinicians who setup, optimise and deliver long-term care of patients needing ventilation. 

The Ventilation service treats patients over the age of eighteen with a variety of problems including: 

  • Neuromuscular diseases such as Duchene Muscular Dystrophy, Myotonic Dystrophy, Motor Neurone Disease, Charcot Marie - Tooth, Spina Bifida and Post Polio Disorders
  • Restrictive respiratory disorders such as Kyphoscoliosis and Post Tuberculosis Thoracoplasty
  • Obesity Hypoventilation Syndrome
  • COPD and Bronchiectasis in Type 2 Respiratory Failure
  • Prolonged weaning from critical care due a variety of conditions, including patients requiring long term invasive ventilation. 

Services and treatment provided include: 

  • Weaning 

We are a regional centre that accepts patients who are delayed in their ability to wean from mechanical ventilation. We perform a full assessment to determine their suitability then arrange to transfer to our unit. We provide specialist input in a multidisciplinary environment to progressively reduce the ventilatory support with the aim to liberate from full mechanical ventilation and facilitate discharge home. We provide rehabilitation and have specialist therapy input. 

  • Long Term Ventilation 

For patients who require tracheostomy ventilation we provide a comprehensive consultant, physiotherapy and multidisciplinary review to establish their needs and potential. We can provide support, training and discharge planning advice to facilitate a discharge home when feasible or to source long term placement in the community. We are able to set up packages of care and the arrangement of equipment and consumables. We can provide support, training and competency frameworks for care agencies. 

  • Non Invasive Ventilation 

We care for patients temporarily needing NIV to overcome an acute illness. We initiate patients requiring long term NIV and provide on-going care in the community through clinic appointments, outpatient appointments and in patient admission for troubleshooting where needed. We also provide a telephone help line. We have approximately1,200patients using long term NIV in the community. 

  • Spinal Injuries (North West Regional Spinal Injuries Centre) 

We have a service level agreement with the North West Regional Spinal Injuries Centre. We offer over sight of their weaning and the long term ventilation of spinal patients. There are 9 ventilation beds with 1 to 2 nursing and a specialist ventilation physiotherapy team. Opportunity to round there in lieu of the VIC is available. 

Facilities 

The Ventilation unit is a large, modern facility with a combination of bays and individual side rooms with bathroom facilities and tracking hoists at a number of bed spaces.There are 12 bed spaces

  • CCT in respiratory medicine and GIM (General Internal Medicine) (or within 6 months)
