Advanced Clinical Pharmacist, Intermediate Care and Specialist Services – Urgent Community Response Team (Maternity cover)
An exciting opportunity has arisen for an enthusiastic and motivated clinical pharmacist to provide Medicines Management support to community specialist nursing teams.
The post holder will work as an advanced clinical pharmacist supporting with queries and medication reviews for the Urgent Community Response Team.
The post holder will complete medication reviews for patients to support them to get the best possible outcomes from their medicines. This will involve analysing and interpreting highly complex clinical situations and communicate effective medicines optimisation solutions. They will support an integrated approach to medicines optimisation, helping to ensure patients have access to the Medicines Optimisation support they require to improve patient outcomes. The post holder will work and communicate effectively within a multi-disciplinary team.
The postholder will provide specialist information, education, training and advice to members of the multi-disciplinary team to maximise pharmaceutical care.
You will receive clinical supervision and peer support from a large Medicines Management team within Merseycare NHS Foundation Trust.

· Work closely with the multi-disciplinary team.
· Support with queries and Domiciliary Medication Reviews for patients under review by the Urgent Community Response Team. This will also include supporting existing Medicines Management pharmacists and technicians already embedded within these teams.
· Support with safe and effective prescribing of medicines within the Urgent Community Response Team.

Qualifications
Essential criteria
- Master of Pharmacy Degree, or equivalent qualification
- Postgraduate diploma in Clinical Pharmacy, or equivalent experience
- Independent prescriber qualification, or willingness to work towards
- Registration with General Pharmaceutical Council
Desirable criteria
- Advanced Clinical Practice qualification, or willingness to work towards
- Membership of the Royal Pharmaceutical Society (RPS)
- Leadership or management qualification/training
Knowledge/Experience

- Experience of working within pharmaceutical services
- Experience of working within an NHS organisation
- Experience of working in hospital pharmacy
- Experience of multi-disciplinary team working
- Experience of undertaking audit within pharmaceutical services
