Understands and acts in line with NMC professional standards for practice contained within the Standards of proficiency for Nursing Associates.

To assess physical and psycho–social needs, plan, manage, implement and evaluate individual programmes of patient care in the most efficient and clinically effective way, which meets the needs of the patient and his/her carer(s).

Uses a range of assessment tools pertinent to the person's need to inform the assessment and assess risk for both patients and staff. Performs continence assessments.

Demonstrates the ability to effectively and reasonably assess input and apply information and data using a range of methods including digital technology and share appropriately with interdisciplinary teams

Understands when to seek appropriate advice to manage a risk and avoid comprising quality of care and health outcomes acting in line with local and national organisational frameworks, legislation and regulations to report risk, and implement actions as instructed.

Demonstrates an ability to prioritise and manage their own workload and recognise where elements of care can safely be delegated to other colleagues carers and family members

Works in close liaison with nursing colleagues, of members of the multidisciplinary team and social care and voluntary sector.

Contributes to the provision of care for people including those with routine non problematic urinary catheters and including those with complex needs that have been taught intermittent self catheterization by a registered nurse. To perform second attempt at trial without catheter patients known to be uncomplicated

Exercise professional accountability in ensuring the safe administration of medicines in line with trust policies and those receiving care

Ensuring information is recorded objectively and reported back to the Bladder & Bowel Service   

Recognise and respond to adverse or abnormal reactions to medications, and when and how to escalate any concerns to coordinator/ team leader.

Undertakes  storage, and disposable disposal of medical products in line with trust policies

Ability to plan ahead for potential scenarios to ensure anticipatory care needs are understood and met.

Is able to show an awareness of the roles, responsibilities and scope of practise of different members of the nursing and inter multidisciplinary team and their own role within it

Ability to recognise when a person's condition has improved or deteriorated by undertaking health monitoring interpret, promptly respond, share findings, and escalate as needed

Collaborate effectively with other members of the multidisciplinary team or other agencies involved in the persons care. Offers support with training in the care home environment for healthcare support workers.

Participates in case conferences, MDTs and care package reviews providing health related information when requested.

Provides support and education in the delivery of care and delegates work appropriately to junior team members. Undertakes staff appraisals, when appropriate.

Promotes and applies the aims and principles of health promotion protection an improvement and the prevention of ill health when engaging with people

Protect health through understanding and applying the principles of infection prevention and control including communicable disease surveillance and antimicrobial stewardship and resistance.

Encouraging and support patient independence and self-care of their condition where appropriate. whilst minimising risk for both patients and staff, making use of all available resources.

Ensures own mileage form and extra duty forms are submitted to the Team Leader within the agreed time frame.

Engagement with appraisal and the development and activation of a personal development plan

Attends mandatory training and other clinical training essential to the role. This includes undertaking e- learning.

Can demonstrate the achievement of competencies required for this post and within a specified time frame. 

Participates in research and audit as required by the Trust.

Participates in and attends staff meetings / working groups as required.

Assist in the development of registered and non-registered staff using supervision, management supervision, appraisal and personal development plans for delegated staff, taking into account both the needs of the service and the aspirations of the individual
