- Part time - 22.5 hours per week (Your paid hours will be 22.5 hours per week. These hours may be done solely in Primary Care or split between the Trust and Primary Care, depending on experience and individual agreement. These hours will be routinely worked between 8am – 6pm Monday to Friday depending on service requirements. Weekends currently operate between 9am – 4.30pm and are currently rostered on a 1 in 6 basis. Staff working solely in Primary Care will usually work weekends within the PCN/GP service and may also be required to support bank holiday cover 9am-5pm (1-2 per year) should this be required by the service. Split posts may work weekends in the acute trust or PCN depending on individual agreement and patient or service need. Split posts will be required to support the current department rota for working 9am-5pm Bank Holidays (usually 1-2 per year) in the acute trust. Working patterns are subject to change to meet patient and service need. It is a requirement of PCN roles to enrol on the CPPE Primary Care Pharmacy Education Pathway if not already completed.)

Job overview
This is an exciting opportunity for a motivated Pharmacist to join our Integrated Pharmacy workforce providing a service to our PCNs within Wallasey and Moreton and Meols.
These PCNs are a group of 8 GP practices working together to provide joined up services to support their patients to live happier, healthier lives. You will join an experienced and dynamic team that includes GPs, Nurses, Health & Wellbeing Coaches, Care Coordinators, Social Prescribers & Physiotherapists to provide clinical pharmacy services to our local population.
We have embraced collaboration to achieve a system wide overview of the patients’ medicines journey. This helps you identify how processes can be improved when patients move between care settings and become part of the system wide pharmacy family. You will be supported to complete the CPPE pathway
Main duties of the job
You will support these PCNs with the delivery of safe, effective systems for medicines optimisation, reducing waste and maximising patient outcomes through structured medication review.
You will contribute to new ways of working and using the skills that your role can bring to evolving PCN services.
You will be responsible for promoting good practice in line with clinical and operational guidelines and will clinically support the pharmacy technicians with medicines reconciliation post discharge and other medicines projects.
On a day-to-day basis you will spend time within the PCN hubs and in GP practices to answer clinical queries from patients, local GPs and clinicians, trusts and other services such as care homes. You will be supported to build strong relationships with community pharmacy and trust pharmacy colleagues.
Ultimately your role will be to support our patients to help them get the most out of their medicines and to play an active part in their own healthcare decisions.

Qualifications, Specific Experience & Training
Essential criteria
- Vocational Master Degree in Pharmacy plus 1 year pre-registration experience
- Postgraduate Pharmacy qualification or equivalent vocational experience
- Post registration clinical experience in a relevant area of practice
- Significant relevant and ongoing CPD
- Experience of multidisciplinary working to deliver a range of complex developments
