- Full time - 37.5 hours per week (Your paid hours will be 37.5 hours per week. These hours may be done solely in Primary Care or split between the Trust and Primary Care, depending on experience and individual agreement. These hours will be routinely worked between 8am – 6pm Monday to Friday depending on service requirements. Weekends currently operate between 9am – 4.30pm and are rostered on a 1 in 6 basis. Staff working solely in Primary Care will usually work weekends within the PCN/GP service and may also be required to support bank holiday cover 9am-5pm (1-2 per year) should this be required by the service. Split posts may work weekends in the acute trust or PCN depending on individual agreement and patient or service need. Split posts will be required to support the current department rota for working 9am-5pm bank holidays (usually 1-2 per year) in the acute trust. Working patterns are subject to change to meet patient and service need. It is a requirement of PCN roles to enrol on the CPPE Primary Care Pharmacy Education Pathway if not already completed.)

Job overview
This is an exciting opportunity for an experienced pharmacy technician to join our Integrated Pharmacy workforce providing a service to our PCNs within Wallasey and Moreton and Meols.
These PCNs are a group of 8 GP practices working together to provide joined up services to support their patients to live happier, healthier lives. You will join an experienced and dynamic team that includes GPs, Nurses, Health & Wellbeing Coaches, Care Coordinators, Social Prescribers & Physiotherapists to provide clinical pharmacy services to our local population.
We have embraced collaboration to achieve a system wide overview of the patients’ medicines journey. This helps you identify how processes can be improved when patients move between care settings and become part of the system wide pharmacy family. You will be supported to complete the CPPE pathway.
Main duties of the job
The role will support the PCN clinical pharmacists to ensure the delivery of safe, effective systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes:
- Be responsible for the development of a better understanding of the principles of medicines optimisation throughout the practice teams and promote good practice in line with clinical and operational guidelines.
- Perform medicines reconciliation for patients discharged from hospital including counselling on medicines use and referral to other clinicians as necessary.
- Communicate with a wide range of healthcare professionals within the trusts, practices and other services such as care home staff.
- Build strong relationships with community pharmacy colleague.
- Support patients with their medication and provide advice regarding benefits, risks and side effects.
- Support improvements in patient care and the development of integrated pathways across primary and secondary care.

As part of the WUTH Integrated Pharmacy Service that supports a number of PCNs within Wirral, these roles are based within Wallasey Wellbeing and Moreton & Meols PCNs but may also support our other PCNs as required by service need. There are opportunities to work with colleagues from the other PCN teams in Wirral as well as having access to the Acute Trust specialists which helps you develop a better understand the full patient journey.

Qualifications, Experience and Training
Essential criteria
- NVQ Level 3 in Pharmaceutical Sciences/BTEC in Pharmaceutical Sciences or equivalent
- Registered with the General Pharmaceutical Council of Great Britain
- Experience working in a pharmacy
- Experience of pharmacy labelling and stock control systems
Desirable criteria
- Accredited accuracy checker
- Primary care pharmacy experience
- Use of EMIS Clinical system
Knowledge and Skills
Essential criteria
- Interpret and dispense prescriptions accurately
- Good written communication skills
- Word processing, e-mail, accurate data entry skills
- Effectively provide advice to patients regarding medicines
- IT literate
